임신이나 수유 중 엄마가 섭취한 음식과 자녀의 알레르기질환 발생이나 예방 효과

ABSTRACT

The increasing prevalence of allergic diseases in the past decades has been caused by environmental rather than genetic factors. Recent research has focused on the relationship between the mother's environmental exposure, especially during pregnancy, and allergic outcomes in the offspring. Intervention studies to prevent allergic diseases have also been conducted. Environmental factors, such as diet induce changes in the mother's intestinal flora, and the signals generated have a permanent effect on the fetal immune development through an epigenetic mechanism, which may be involved in disease development. This review outlines whether dietary patterns in mothers are related to allergic diseases, and it summarizes whether supplementation with micronutrients, such as omega-3 unsaturated fatty acids, vitamin D, and folic acid can prevent allergic diseases. Studies have found that a westernized diet is associated with an increase in the prevalence of allergic diseases; however, intervention studies conducted with micronutrients or probiotics do not provide clear results regarding its preventive effect. A mother's diet during pregnancy or lactation may affect the health of the child. Well-designed intervention studies are warranted to attempt to prevent allergic diseases. Consequently, good dietary patterns based on high-quality evidence are recommended for pregnant and lactating mothers.

서 론

임신을 하게 되면 임산부는 태아가 건강하게 태어나길 바라는 마음으로 자신의 생활패턴을 바꾸는 경향이 있다. 예를 들면 태아에 안 좋다고 하면 좋아하던 커피와 같은 기호 식품도 피하게 되고, 금연을 하거나 금주를 한다. 태아는 발달과정에 있으면서 아빠의 영향도 있지만, 탯줄을 통해 엄마와 밀접하게 연결되어 있어서 유전적으로 뿐 아니라 엄마와 관련된 환경 인자의 영향을 많이 받을 수 있다. 이런 영향은 출생 후 환경과 연관된 질환 발생의 위험 요소가 될 수 있다(Kwon & Kim, 2017).
알레르기질환은 지금까지 지속적인 유병률의 상승을 보여오고 있다. 20세기 후반에 천식의 유병률은 2-3배의 증가가 나타났고, 전 세계적으로 천식 환자가 3억명에서 2025년에는 4억명으로 증가될 것으로 예측한다(Pawankar, 2014; Custovic, 2017). 유병률 상승의 원인을 환경적 요인에서 찾고 이를 예방함으로써 유병률이 감소할 수 있다는 생각에 연구들이 시행되고 있다(Annesi-Maesano et al., 2021; Cecchi et al., 2018). Strachan (1989)이 역학연구들의 결과를 통해 소아기 때 감염 질환과 연관된 Th1의 발달 감소가 알레르기질환과 관련된 Th2의 반응의 증가를 이끌 수 있다는 ‘위생가설’을 제시한 이후 중재연구나 원인규명 연구들은 출생 시 Th2로 편향된 면역계가 환경에 노출되면서 Th1의 발달로 균형을 맞추거나 조절 T 세포의 능력 증가에 영향을 주는 환경 인자에 대한 관심이 증가하였다. 위생가설은 2000년대가 되면서 메타노믹스(Lepage et al., 2013)의 발달로 Th1 면역반응의 발달이나 조절 T세포 반응을 유도하는 장내세균총의 변화가 알레르기질환이나 알레르겐의 감작과 연관된다는 연구 결과들을 통해 ‘미생물총 가설’로 진화하였다(Rautava et al., 2005). 장내세균총에 영향을 줄 수 있는 인자로는 섭취하는 음식이 있다. 알레르기질환의 유병률이 높은 곳은 서구화된 선진국으로 서양식 식단이 원인 중 하나로 제시된다(Thorburn et al., 2014). 서양식 식단은 높은 지방과 당이 포함되어 있고, 일반적으로 간편하고 고도로 정제되고 가공된 식품 등이 포함된다(Guilleminault et al., 2017). 이에 반해 건강식단은 ‘지중해식 식단’으로 과일과 채소에 많은 섬유질을 섭취하고 올리브 오일과 생선 등의 섭취가 높은 음식으로 구성되어 있다. 서양식 식단은 비만식이이고, 많은 역학연구들을 통해서 심뇌혈관계 질환뿐 아니라 천식과 같은 알레르기질환이나 자가면역질환 발생과 관련성이 있고(Thorburn et al., 2014), 지중해식 식단은 천식 발생에 예방 효과가 있거나 천식 증상 조절에 영향을 준다고 보고하였다(Garcia-Marcos et al., 2013; Wood et al., 2012; Zhang et al., 2019). 그리고 임신 전부터의 식이패턴의 변화나 프로바이오틱스와 프리바이오틱스의 섭취에 따른 장내세균총의 변화가 알레르기질환과 어떤 연관성을 갖는지 확인하고, 중재연구를 통해 알레르기질환의 예방 효과가 있는지 보는 연구들도 계속적으로 보고되고 있다(Hendaus et al., 2016; Nuzzi et al., 2022; Prescott et al., 2011).
이 종설은 지금까지 보고되고 있는 임신 기간 중 엄마가 섭취한 식이 패턴과 관련된 인자와 알레르기질환과의 연관성 및 중재연구 결과를 정리해보고자 한다.

본 론

1. 임신 전이나 임신 중 엄마의 식이와 자녀의 알레르기 질환 연관성

임신 중 엄마가 섭취하는 서양식 식단은 태어나는 자녀의 질환 발생과도 연관된다.
임신 기간 중 엄마가 섭취한 식단이 자녀의 알레르기질환 발생에 영향을 줄 수 있음을 보고한 출생코호트 연구에서 특히 눈에 띄는 결과는 야채 섭취량의 감소가 자녀의 천식과 같은 알레르기질환의 위험률이 높아짐과 연관된다는 것이다(Erkkola et al., 2012; Miyake et al., 2010a). 2015년에 발표한 인구 기반 후향적 코호트 연구에서는 임신 중 패스트푸드 섭취량과 비례하여 자녀의 3.5세 때 천식 발생 위험이 증가하며, 많이 먹을수록 천식 증상에 대한 상대 위험률이 증가하였다. 또한 비염 증상에 대한 위험률도 증가함을 보고하였다(von Ehrenstein et al., 2015). 체계적 문헌고찰과 메타분석 연구들에서 임신 기간 중 엄마의 식이패턴, 식품군, 비타민, 미량원소와 알레르기질환 간의 상관성을 보았을 때 식이패턴이나 식품군과의 연관성보다는 미세영양소와 알레르기질환과의 연관성을 볼 수 있었다(Beckhaus et al., 2015; Venter et al., 2020). 미세영양소는 주로 비타민과 오메가 3로, 소아기 천명이나 천식 발생을 낮추는 연관성이 비타민 D와 오메가 3에서 보였고, 통계적인 유의성은 없었으나 알레르기비염 발생을 낮추는 경향을 오메가 3에서 보여주었다. 그러나 두 영양소는 아토피피부염이나 음식알레르기의 위험률을 낮추지는 못 했다고 보고하였다. 이런 연구 결과들은 임신 기간 중 섭취한 식이패턴이나 미세영양소가 일부 알레르기질환 발생이나 예방에 영향을 줄 수 있음을 알려준다. 이를 바탕으로 중재 연구에 대한 관심이 높아지고 관련 연구 결과들이 계속적으로 발표되고 있다.

2. 임신 전이나 임신 중 엄마의 식이 중재를 통한 알레르기 질환 예방 결과

1) 미세영양소와 알레르기질환의 관련성(Table 1)

Vitamin D · Based on its epidemiological association with allergic disease in observational studies, vitamin D is considered an important target candidate for preventing allergic disease in intervention studies.
· Interventions with vitamin D have not shown consistent or long-term benefits. For instance, a prospective prebirth cohort study, which confirmed the effect of vitamin D at two different times, showed a reduction in the incidence of asthma or recurrent wheezing by 6.1 % at 3 years of age, but not at 6 years (Litonjua et al., 2016, 2020).
ω-3 PUFA or fish oil · Given the numerous epidemiological studies that showed low ω-3 PUFA level is a risk factor for asthma, it can be an interesting target as a preventive strategy against allergic disease.
· Preventive effects of ω-3 PUFA or fish oil against allergic disease during the postnatal intervention period was not demonstrated in the 2016 Cochrane review. However, results from several intervention studies on mothers suggested that the benefits may be greater during pregnancy.
· Although the preventive effect of ω-3 PUFA has remained controversial, two recent meta-analyses showed that it had no effects on reducing allergic outcomes.
Folate · Folate intake during early pregnancy has been shown to reduce the risk of neuronal tube malformation; however, many studies have shown that it can be a risk factor for asthma or recurrent wheezing among infants and young children.
· Although several studies have shown that folate was not notably associated with asthma or had protective effects against allergic disease, a recent systemic review and meta-analysis suggested that folic acid affected the development of allergic disease, depending on the ingested dose, timing, and intake mode, such as supplements or fortified foods with folic acid (Chen et al., 2021).
Probiotics · Since the first study using Lactobacillus rhamnosus GG in a randomized human trial (Kalliomä ki et al., 2003), several studies have investigated the protective effects on allergen sensitization and skin allergy, especially atopic dermatitis. However, these results are still unclear and appear to depend on the strain, timing, method of administration, host, and other environmental factors.
· Several meta-analyses have shown that probiotics significantly prevented the occurrence of infant eczema when administered to mothers during pregnancy and lactation; however, it did not demonstrate preventive effects against allergic diseases, such as asthma and rhinitis.

PUFA, polyunsaturated fatty acids.

(1) 비타민 D

알레르기질환이 증가하고 있는 것처럼 20세기 이후로 비타민 D 결핍도 증가하는 경향을 보이고 있다(Basatemur et al., 2017). 비타민 D는 골대사와 칼슘 항상성에 중요한 역할을 하는 요소이지만, 많은 연구들에서 시기에 따라 다른 형태로 면역 중재 기능을 한다고 보고하고 있다(Lange et al., 2009). 실험 연구들에서 신장세포 이외 상피세포나 각질형성세포, 활성된 거대포식세포와 수상돌기세포에 비타민 D 활성에 필요한 1α-hydroxylase가 표현되며(Adams & Hewison, 2008), 선천 면역과 획득 면역 기능을 조절하는 기능을 하고(Liu et al., 2006; Sigmundsdottir et al., 2007), 태아 시기에는 폐와 면역계의 발달 과정에 중요한 역할을 한다(Evans et al., 2006; Hart et al., 2015; Wang et al., 2020). 비타민 D 결핍은 전세계적으로 임산부에 매우 흔한 것으로 알려져 있고, 임산부의 비타민 D 결핍의 유병률은 영국에서는 21.2%, 벨기에는 44.6%, 중국은 83.6%로 나라와 고도에 따라서 다르다(Hoxha et al., 2014). 우리나라의 경우 대규모 연구가 없어 정확하게 산모의 비타민 D 결핍 유병률을 알 수 없지만 한 병원의 연구 결과 비타민 D 농도 20 ng/mL 미만을 비타민 D 결핍으로 정의했을 때 77.3% (건강한 임신하지 않은 여성 79.2%), 10 ng/mL 미만의 중증 결핍은 28.6% (건강한 임신하지 않은 여성 7.2%)였다(Choi et al., 2015). 이 연구 결과를 통해 우리나라에서 임신 시에 중증 비타민 D 결핍의 유병률이 증가할 가능성을 유추해 볼 수 있다.
엄마의 비타민 D 농도가 낮으면 탯줄로 연결된 태아의 비타민 D 농도도 영향을 받을 수 있다. 혈중 비타민 D 농도를 임산부와 출생 시 신생아에서 검사했을 때 30 ng/mL 미만인 경우가 임산부가 54%, 신생아는 75%에서 나타났다고 보고하였고, 제대혈의 비타민 D 농도는 엄마 혈중농도의 60% 정도를 보이고, 신생아의 절반에서 20 ng/mL 미만을 보였다고 한다(Noriega & Savelkoul, 2021). 분자생물학 관점에서 임신 기간 중 엄마가 비타민 D를 섭취하게 되면 탯줄혈액에서 immunoglobulin-like transcript 3과 4의 mRNA가 증가한다. 이 물질은 T 세포의 무반응(anergy)을 유도하는 기능을 하는 것으로 알려져 있다.
임신 중 엄마의 낮은 비타민 D 농도는 태아의 폐와 면역의 발달을 저해하는 영향을 줄 수 있기 때문에 엄마가 비타민 D 복용으로 충분한 농도를 유지하면 면역과 관련된 자녀의 질환 발생을 예방할 수 있음을 가정해 볼 수 있다.
임신 기간 중 엄마나 출생 후 신생아에게 비타민 D를 섭취하게 하여 알레르기질환을 예방하였는지에 관한 중재 연구들 결과를 보면 많은 연구들에서 비타민 D의 알레르기질환의 예방 효과가 있음을 입증하였지만(Camargo et al., 2007; Devereux et al., 2007; Erkkola et al., 2009; Litonjua et al., 2016; Miyake et al., 2010b), 일부 연구에서는 상반된 결과를 보여주고 있다(Chawes et al., 2016; Litonjua et al., 2020; Visness et al., 2015; Wegienka et al., 2015). 자녀가 천식의 위험성을 가진 엄마를 대상으로 30 ng/mL 이상의 비타민 D 농도를 유지하도록 했을 때 자녀의 천식이나 천명에 대한 예방 효과를 본 VDAART (The Vitamin D Antenatal Astham Reduction Trial) 코호트 연구 결과에서는 임신 초기와 후기에 비타민 D 농도가 30 ng/mL 이상을 유지했던 경우에 자녀의 천식이나 천명을 예방하는 효과가 유지되었음을 보고하였다(Lu et al., 2021). 그렇지만 Gale 등(2008)의 연구에서는 임신 기간 중 비타민 D의 농도가 30 ng/mL 초과인 엄마에서 태어난 자녀는 9개월에 습진, 9세에 천식의 위험률이 높았다고 보고하며, 임신 기간 중 비타민 D 복용은 모든 산모에게 필요하지 않고 비타민 D 결핍이 있는 임산부에게만 해야함을 주장했다. 또한 최근 임신 시 비타민 D 복용과 관련된 알레르기질환 예방 효과에 대한 메타분석 결과에서도 아직은 예방 효과를 찾지 못하였다(Luo et al., 2022).
비타민 D는 태아의 면역 발달에 중요한 역할을 하기 때문에 알레르기질환을 예방하는 방법으로 제시해 볼 수 있지만, 적절한 시기에 안전하면서 효과를 보일 수 있는 용량과 투여 방법 등에 대해 정의할 수 있기 위해 더 잘 설계된 연구가 더 필요하겠다.

(2) 오메가3 불포화지방산(ω-3 polyunsaturated fatty acids, PUFA)과 피쉬 오일(fish oil)

불포화지방산은 우리 몸에서 생산하지 못하기 때문에 성장과 발달을 위해 반드시 음식을 통해서 섭취해야 하므로 필수지방산이라고도 한다. 불포화지방산은 오메가 3 와 6 두 종류로 구분한다. 주로 세포막에 위치하며 항상성과 세포신호전달체계에 중요한 역할을 하며, 대사과정을 통해 식물성 오일에 많이 함유된 오메가 6는 프로스타글란딘 D2와 E2, 루코트리엔 B4를 생성하고, 생선에 많이 들어있는 오메가 3는 3가 관련 프로스타글란과 루코트리엔뿐 아니라 오메가 6와 달리 항염증반응의 효과를 보이는 리졸빈(resolvin)과 프로텍틴(protectin)을 형성한다(Candela et al., 2011).
현대 식이의 변화에서 가장 많이 변한 것은 포화지방산과 트랜스 지방산 그리고 불포화지방산 중 오메가 6의 양이 증가했으나 오메가 3 양은 감소한 것이다. 특히 두 불포화지방산간의 비율이 과거 식이에는 1-2:1이였으나 현대 식이에서는 10:1 이상으로 눈에 띄게 증가하였다(Candela et al., 2011; Simopoulos, 2011).
많은 역학연구로 오메가 3 불포화지방산이 낮은 식이가 알레르기질환이나 천식의 위험 요소이고 피쉬 오일을 섭취 시 천식과 같은 알레르기질환을 예방할 수 있음 알게 되었고(Hodge et al., 1996), 실험 연구를 통해서 오메가 3 불포화지방산이 항염증 반응에 중요하고, 면역계에서 면역조절반응을 유도할 수 있음을 알게 되었다(Calder, 2007, 2009). 그래서 오메가 3 불포화지방산이 많은 피쉬 오일 보충제를 출생 후 소아기까지 투여하여 알레르기질환을 예방할 수 있는지에 대해 살펴본 17개 연구들을 2016년 코크란 리뷰를 통해서 분석했을 때 불행히도 예방 효과는 보이지 않았다(Schindler et al., 2016). 그러나 임신 기간 중 피쉬 오일 보충제를 엄마에게 투여한 몇몇 연구들에서는 알레르기질환의 예방 효과가 나타났다(Dunstan et al., 2003; Furuhjelm et al., 2009; Sausenthaler et al., 2007). 또한 1990년 무작위 배정 임상시험으로 임신 마지막 주수에 피쉬 오일 보충제를 투여한 군에서 자녀가 18-19세 때 알레르기질환 예방 효과를 살펴보았을 때 천식은 통계적으로 유의하게 감소시켰고(hazard ratio [HR], 0.54; 95% confidence interval [CI], 0.32-0.90; p=0.02), 알레르기 비염은 감소는 하였으나 통계적 유의성은 없었음(HR, 0.70; 95% CI, 0.47-1.05; p=0.09)을 보고하여 임신 기간부터 피쉬 오일 보충제를 투여할 때 일부 알레르기질환의 예방 효과가 상당히 오래 지속될 수 있음을 제시하였다(Hansen et al., 2017).
그러나 2019년 발표한 체계적 문헌고찰에서 10개의 무작위 배정 임상시험을 메타 분석했을 때 오메가 3 불포화지방산 보충제는 자녀의 달걀이나 땅콩 항원의 감작을 감소시키는 경향을 보이나 다른 알레르기질환에 대해서 예방 효과를 보이지 못했다고 보고하였고(Vahdaninia et al., 2019), 2020년에 유럽알레르기학회에서 발표한 체계적 문헌고찰과 메타분석에서도 오메가 3 불포화지방산 보충제는 천식과 알레르기비염에서 예방 효과는 보이지만 통계적인 유의성이 없고(천식: odds ratio [OR], 0.70; 95% CI, 0.45-1.08; 알레르기비염: OR, 0.76; 95% CI, 0.56- 1.04), 아토피나 음식알레르기에서는 예방 효과를 보이지 못했다고 하였다(Venter et al., 2020).
많은 관찰연구와 실험 연구에서 오메가 3 불포화지방산의 알레르기질환 예방 효과를 보여주고 있지만 아직은 질환 예방을 위해 임신 기간 중 사용을 권장하기에는 그 효과가 확실 치 않다.

(3) 엽산(folate, folic acid)

엽산은 오랜 기간 동안 신경관 결손이나 출생 시 기형을 예방하는 효과 때문에 임신 전이나 임신 중 복용하도록 권장하고 있다(De-Regil et al., 2015; MRC Vitamin Study Research Group, 1991). 엽산은 우리 몸에서 생성하지 못하는 수용성 비타민 B9으로 식품을 통해서 반드시 섭취를 해야 한다. 자연적으로 채소류(시금치, 쑥갓 깻잎 등), 과일류(오렌지, 딸기, 토마토 등) 등의 식품에 함유되어 있으나 조리 과정에서 많이 파괴되다 보니 임산부에게 합성 엽산제제(folic acid)나 엽산 강화식품을 먹도록 하고 있다(Chen et al., 2021). 엽산은 우리 몸에서 생화학 반응 시 메틸기를 제공하는 기능을 하기때문에 DNA 메틸화와 관련된 질환, 특히 알레르기질환과의 연관성에 대한 연구들이 있었다(Hollingsworth et al., 2008). 알레르기질환 발생 위험 요소로서 엽산제제의 연관성에 대한 연구 결과를 살펴보면 일부 연구(Martinussen et al., 2012; Crider et al., 2013)는 엽산제제 복용은 알레르기질환과 관련성이 없다고 보고하거나 농도가 높을수록 예방 효과가 있다고 보고하였다(Magdelijns et al., 2011; Matsui & Matsui, 2009). 그러나 많은 연구들은 임신 초기에 엽산제제를 복용하는 것이 자녀에게서 천식이나 아토피피부염 등의 알레르기질환 발생의 위험 인자일 수 있다(Bekkers et al., 2012; Dunstan et al., 2012; Haberg et al., 2009; Kiefte-de Jong et al., 2012; Whitrow et al., 2009)고 보고하였다.
2015년에 26개의 연구들을 대상으로 체계적 문헌고찰과 메타 분석한 연구는 엽산 섭취와 알레르기질환과 연관성이 없었으나 조기 노출 시 출생 1년 이내 천명의 발생과 연관이 있고, 흥미롭게도 엽산 대사 장애를 가진 유전적 소인(TT genotype of MTHFR C677T polymorphism)이 있을 때 엽산 섭취가 천식의 위험률을 높였음(OR, 1.41; 95% CI, 1.07-1.86)을 보고하였다(Wang et al., 2015).
그러나, 2018년에 발표한 인구 기반 코호트 연구 결과에서 엄마가 아토피 소인이 없을 때는 알레르기질환과 연관성이 없으나 아토피 소인이 있는 엄마가 마지막 임신 주기에 엽산을 복용한 경우에 첫 주기에 엽산을 복용한 엄마와 비교할 때 자녀가 3세에 천명에 대한 위험도가 1.67 (95% CI, 1.12-2.49)이고, 지난 1년간 천명의 위험도는 1.88 (95% CI, 1.05-3.34)로 증가하는 것을 보였다(Alfonso et al., 2018). 반면, 최근에 Chen 등(2021)이 발표한 체계적 문헌 고찰과 메타 분석 연구에서는 임신 시 엄마의 엽산 섭취가 자녀의 알레르기질환 발생에 연관이 되며, 특히 엽산 섭취 형태에 따라서 위험률이 달라짐을 보고하기도 했다.
아직도 임신 기간 중 엽산 섭취와 알레르기질환에 대한 연관성을 분명하게 설명하기는 어려우며, 향후 더 많은 연구들을 통해서 엽산의 적정 섭취량이나 투여 시기, 엄마의 상태 등을 고려한 권고사항이 제시되길 기대해 본다.

(4) 프로바이오틱스와 프리바이로틱스(probiotics and prebiotics)

요즘 사람들은 건강을 위해 유산균이나 정장제로 부르는 프로바이오틱스가 포함된 건강보조식품을 많이 찾는다. 프로바이오틱스는 우리 몸의 건강에 효과적이면서 안전한 살아있는 미생물균총을 말하며, 프리바이오틱스는 장관미생물총이 잘 증식할 수 있도록 하는 수용성 섬유소나 올리고당과 같은 것을 말한다(Gibson et al., 2017; Hill et al., 2014).
많은 역학 연구들을 통해서 소아기 감염질환의 감소가 알레르기질환의 유병률 증가의 원인으로 생각되는 ‘위생가설’이 제시되었다. 2000년대에 무균 동물을 이용한 실험연구를 통해서 정상 면역 발달과정에 장관미생물총이 면역계와 작용하여 중요한 역할을 한다는 것을 증명하였고, 전향적 역학연구들은 알레르기질환이나 알레르겐의 감작이 장내세균총의 변화와 연관됨을 보고하였다. 1980년대 제시된 ‘위생가설’은 ‘미생물총 가설’로 재명명되며(Rautava et al., 2005) 연관된 질병 발생이나 중재 효과에 대한 연구들이 진행되고 있다. ‘미생물총 가설’을 바탕으로 출생 후 신생아에게 또는 엄마에서부터 신생아에게로 프로바이오틱스를 투여하여 알레르기질환 특히 아토피피부염과 알레르겐의 감작 유무에 대한 효과를 보는 연구들이 많이 진행되었고, 이후 나이가 들면서 천식이나 알레르기비염 등에 대한 예방 효과가 있는지 지속적으로 관찰한 연구 결과를 보고하고 있다.
처음으로 시행된 이중맹검위약대조임상시험 연구는 2001년에 알레르기질환의 고위험군인 엄마에게 임신 시기와 수유 시기 동안 그리고 출생 후 6개월까지 아이에게 Lactobacillus GG를 투여하였을 때 아토피피부염의 예방 효과가 보였고, 4세까지 효과가 지속되었음을 보고하였다. 그러나 음식알레르기와 같은 다른 알레르기질환의 예방 효과는 찾지 못하였다(Kalliomä ki et al., 2003). 지금까지 발표된 프로바이오틱스의 알레르기질환 치료나 예방 효과에 대한 체계적 문헌고찰과 메타분석 결과를 보면 습진의 위험률을 낮추는 경향이 높지만, 천식과 같은 다른 알레르기질환에 대해서 아직은 프로바이로틱스가 갖는 예방 효과는 크지 않았다고 보고하고 있다. 그러나 알레르기질환 발생 위험이 높은 신생아, 즉 가족 중 알레르기질환 병력이 있는 신생아에서는 2세 이내에는 습진 발생의 예방 효과는 다른 질환보다 크다고 보고하였다(Sestito et al., 2020).
질환 예방 효과를 위해 프로바이오틱스를 투여하는 적절한 시기도 연구에 중요한 고려 사항일 수 있다. 많은 연구들은 임신 마지막 주수인 36주 이상에 엄마에게 투여하기 시작하여 수유 중 지속하는 경우가 많았으며, 자녀에게는 출생 3개월에서 2년까지 투여하였다. 동물실험에서는 출생 전 효과를 보여주고 있지만, 엄마의 프로바이오틱스 투여의 효과에 대해서는 아직 연구들마다 결과가 달리 나타나고 있다. 그래도 Rautava 등(2012)의 연구에서 보면 산전, 수유 중에 프로바이오틱스를 부작용 없이 안전하게 투여하였고, 알레르기질환을 가지고 있는 엄마의 자녀에게 습진의 발생률을 효과적으로 예방하였음을 보고하기도 하였다.
프로바이로틱스의 효과는 투여량과 투여종에 영향을 받으며. 투여 시기에 따라서 효과가 달라질 수 있다. 주로 프로바이오틱스로 사용되어 연구되어진 미생물총은 LactobacillusBifidobacterium이 가장 많고, Propionibac-terium, Streptococcus, Lactococcus, Escherichia coli를 병용투여하여 시험되고 있다. 연구들에서 투여한 균주양은 하루에 1×107에서 1×1010 집락형성단위(colony forming unit, CFU)를 보였는데, 효과적인 최소 투여량으로 음식이나 보조제에 들어있는 살아있는 균주 총수가 1×109 CFU/day 정도는 되어야 한다고 권고된다(Lopez-Santamarina et al., 2021). 요즘은 프로바이오틱스 제제를 만드는 기술에 나노테크놀로지를 적용하여 효과를 더 높이고자 하는 경향을 보인다(Durazzo et al., 2020). 이런 부분들이 향후 프로바이오틱스의 알레르기질환 예방 효과에 어떤 영향을 줄지는 지속적인 연구가 필요할 것이다.

2) 알레르기가 잘 발생하는 음식의 제한의 알레르기 질환 예방 효과

최근에 이유식과 관련된 권고사항이 바뀌었음에도 불구하고 오래전부터 알레르기질환을 예방하는 방법으로 임신 시나 모유 수유 시 엄마가 알레르기 성향이 높다고 알려진 견과류와 땅콩, 계란, 우유, 생선 등을 안 먹거나 자녀에게 고형식을 늦게 먹이는 방법으로 이유식을 6개월 이후에 시도하는 방법은 아직도 많은 사람들이 실천하고 있다. 그러나 미국이나 유럽의 소아영양과 알레르기 전문가그룹(Agostoni et al., 2008; Greer et al., 2008; Host et al., 2008)은 임신 기간이나 수유 중 모체의 식이제한이 알레르기질환의 발생을 줄이지 못해서 권고하지 않는다고 하였다. 그리고 2012년 코크란 리뷰(Kramer & Kakuma, 2012)에 따르면 알레르기질환 발생 위험이 높은 아기를 임신한 산모의 식이제한이 알레르기질환의 발생을 예방하는 것보다 재태 체중 증가가 잘 안되거나, 미숙아 출산과 관련됨을 보고하였다.
그리고 2015년에 발표된 The Learning Early about Peanut allergy 연구(Toit et al., 2015)는 4-11개월 중증 아토피 피부염이나 달걀알레르기 영아를 대상으로 5년간 땅콩을 섭취하거나 제한을 했을 때 땅콩알레르기 발생률을 보았다. 연구 결과 5세 때 땅콩알레르기 발생률은 땅콩 제한군에서는 35.3%였고, 땅콩을 섭취한 군은 10.6%였다. 이 연구 결과의 의미는 알레르기질환 위험을 가진 아이들에게 1차 알레르기질환 예방에 있어서 식이 제한보다는 조기 알레르겐에 노출이 예방 효과가 더 높음을 알려주는 것이다.
이런 연구들을 통해 무분별한 식이 제한은 알레르기질환의 예방 효과는 거의 없고 모체나 태아 그리고 영아에게 여러 면에서 부정적 측면이 있을 수 있음을 기억해야겠다.

3. 식이나 영양소가 알레르기질환에 영향을 주는 기전

임신 기간 중 모체의 면역계는 Th 2와 비슷한 사이토카인의 농도가 높고 조절 T세포가 많음을 보여준다(Jenmalm, 2017). 또한, 아토피 소인이 있는 산모에서 Th 2 편향성이 높고, 신생아 면역계가 Th 2 편향을 보여준다(Sandberg et al., 2009). 심지어 이런 편향성이 영아 후기까지 지속 시 알레르기질환의 발생률이 증가함을 보고하기도 했다(Abelius et al., 2011). 태아의 면역계는 임신 주수 10-12주부터 발달을 시작하며(Haynes et al., 1988), 임신 주수 2기에는 항원에 반응할 수 있는 능력을 갖춘다(Papiernik, 1970; Rechavi et al., 2015). 엄마를 통한 환경 인자의 노출에 태아의 면역계는 반응하며 출생 후 신생아 혈액 내 T 세포가 5%-10%까지 기억세포나 성숙한 작용세포로 분화되어 있는 것을 관찰할 수 있다(Jenmalm, 2017; Zhang et al., 2014).
환경과 관련된 만성 질환의 역학연구를 통해서 David barker가 주장한 ‘건강과 질환의 발달과 관련된 기원(developmental origins of health and disease)’이라는 가설 속 태아프로그래밍(Fetal progamming)이라는 개념을 생각해 볼 필요가 있다. 발달을 하고 있는 태아 시기는 면역학적 취약 시기로 모체를 통한 환경 인자의 노출은 후생기전을 통해 유전자 표현을 변화시키기도 하지만, 질환의 감수성(susceptability)도 증가시키게 된다는 것이다(Kwon & Kim, 2017). 모체의 서구화된 식이나 미세영양소는 태아의 면역계 발달에 직접적으로 또는 모체의 장관 미생물총의 변화를 통해 모체의 면역계에 영향을 주고, 편향된 신호들이 후생기전을 통해 태아에게 영향을 주어서 태아프로그래밍을 유도할 수도 있다. 그러므로 알레르기질환을 예방하기 위해서 모체에서부터 올바른 식이와 적절한 미세영양소나 프로바이오틱스 등의 공급은 태아에 전달되는 환경 인자에 대한 균형을 맞출 수 있는 신호 전달 체계를 이끌어 태아 및 자녀의 알레르기질환을 예방하는 기전으로 작용할 것으로 생각한다.

결 론

알레르기질환의 유병률 증가 원인은 유전적 원인보다 환경 요인의 영향이 크고, 서구화된 식이패턴의 변화는 중요한 하나의 원인이다. 특히 엄마가 섭취한 식이는 모체의 장관세균총에 영향을 주며, 태아에게는 후생기전을 통해 면역 발달에 영구적인 영향을 주며 질환의 발생에 관여할 수 있다. 알레르기질환과 관련된 식이패턴은 지방과 당의 함유가 높은 서구화 식단이나 피쉬 오일에 많이 든 오메 가 3 불포화지방산, 비타민 D, 프로바이오틱스나 프리바이오틱스, 임신 기간 중 신경계 발달 이상을 예방하기 위한 엽산 등이 제시된다. 태아와 영아의 면역계가 취약한 시기에 올바른 식이나 부족한 영양소의 제공은 알레르기질환을 일차적 예방할 수 있는 방법으로 생각해 볼 수 있다. 그러나 지금까지 미세영양소의 제공을 통한 중재 연구들이 아직은 일정한 결과를 주지 못 한다는 점을 보면 각각의 미세영양소들이 결국은 함께 관여하면서 영향을 줄 수 있을 것 같다. 그래서 오히려 미세영양소의 제공보다는 올바른 식이습관을 임신 기간 중 가지도록 교육하는 게 지금으로서는 중요하지 않을까 생각한다. 향후 이런 부분에 대한 지속적인 연구를 통해서 임산부 및 수유모들에게 알레르기질환을 예방할 수 있는 식이에 대한 가이드가 만들어지길 기대해 본다.
