코로나 팬데믹 시기 난임 치료 참여자 대상 연구에 관한 스코핑 리뷰

ABSTRACT

Purpose

This study aimed to conduct a scoping review to understand how the coronavirus disease 2019 (COVID-19) pandemic has affected patients under infertility treatment.

Methods

The 5 steps of the scoping review by Arksey and O'Malley were applied: (1) identifying the research question; (2) identifying relevant studies; (3) study selection; (4) charting the data; and (5) collating, summarizing, and reporting the results. Using 2 databases (PubMed and Cochrane Library), studies on COVID-19 and infertility treatment were searched, and 13 articles were selected for analysis.

Results

The studies were conducted in North America (5 articles), Europe (3 articles), Asia (2 articles), and other regions (3 articles). The academic fields of the published journals were primarily reproductive medicine (10 articles), followed by psychiatry and psychology (2 articles), and complementary medicine (1 article). Regarding the research topic, studies on stress and anxiety were the most common (7 articles), followed by pregnancy rates (3 articles), and pregnancy planning or treatment decisions (3 articles). The COVID-19 pandemic had a negative effect on stress and anxiety of patients in 5 out of 7 articles, no changes in pregnancy rates in 3 out of 3 articles, and negative effects on pregnancy planning or treatment decisions in 2 out of 3 articles.

Conclusion

The results of this review suggest that evidence-based information on patients with infertility is needed to prevent unnecessary anxiety, stress, and treatment delays in the upcoming postpandemic transition period.

서 론

우리나라의 저출산 문제는 다른 경제협력개발기구(Organisation for Economic Co-operation and Development, OECD) 국가들과 비교해도 유례없는 수준이다. 「2021년 출생통계」에 따르면, 2021년의 합계출산율은 0.84명으로 2018년 이후 1명에 미치지 못하고 있으며, OECD 회원국의 평균 합계출산율이 2020년 기준 1.59명인 것을 감안한다면 매우 낮은 수준이다(Statistics Korea, 2022). 저출산에 대한 대응방안 중 하나로, 난임에 대한 사회적 관심이 높아졌고 치료에 대한 건강보험 급여화가 추진되면서 기존에 비급여로 적용된 난임 치료 시술은 표준화과정을 거쳐 2017년 10월부터 체외수정과 인공수정에 대해 건강보험을 적용하였다(Ministry of Health and Welfare, 2022).
동시에, 우리사회는 2020년 1월부터 코로나바이러스감염증-19 (coronavirus disease 2019, COVID-19)로 인한 팬데믹(이하 코로나 팬데믹)을 겪으며 지난 약 3년여의 기간 동안 많은 변화를 경험했다. 코로나 팬데믹 기간에 합계출산율도 감소하는 경향을 보였는데, 2021년 0.81명, 2022년은 0.77명이며, COVID-19의 장기영향추계에서는 2025년 합계출산율이 0.52명까지 감소할 것으로 전망되기도 했다(Statistics Korea, 2021). 이러한 상황에서도 난임 치료를 위한 환자 수는 건강보험 급여화 이래 증가 추세이다. 2017년 여성불임(N97)으로 진료를 받은 환자 수는 연간 약 146천명에서 2020년 약 149천명, 2021년 약 163천명으로 코로나 팬데믹 상황에서도 증가하였고, 요양급여비용총액도 2020년 약 1,626억원에서 2021년 2,051억원으로 약 1.26배 증가하여 난임 치료에 대한 수요는 코로나 팬데믹 시기에 오히려 확대되었다(Health Insurance Review and As sessment Service, 2023). 이처럼 코로나 팬데믹 기간에 우리사회는 전반적인 의료이용의 감소 및 출산율 저하를 경험하고 있지만, 난임 치료에 대한 필요도가 있는 가구는 증가하고 있는 상황이다.
코로나 팬데믹 이전의 상황을 반영하는 2020년 이전의 난임 치료에 관한 국내외 연구동향을 살펴보면, 국내에서는 주로 난임 치료를 받는 참여자의 스트레스, 불안, 우울 등 심리적 특성(Jung & Kim, 2017; Kim et al., 2016; Kim, 2018; Kim et al., 2021) 및 보조생식술을 받는 여성의 난임관련 삶의 질(Lee et al., 2020)을 측정했고, 난임 여성의 임신과 관련된 요인으로 생활습관에 초점을 두어 신체활동(Han & Kim, 2021), 비만의 위험(Kim & Han, 2021)을 밝힌 연구들이 수행되었으며, 나아가 난임 치료 시술에 따른 비용 연구(Kim & Hwang, 2016; Moon & Jeon, 2022), 난임 환자의 한방 치료 등 보완의학 이용에 대한 연구(Hwang et al., 2019; Kim et al., 2018)가 보고되었다. 국외연구에서도 난임 환자의 스트레스, 불안, 우울 및 수면, 삶의 질에 관한 연구가 다수 발표되었고(Cheng et al., 2018; Freeman et al., 2018; Massarotti et al., 2019; Nicoloro-Santa Barbara et al., 2018), 비만, 영양 등 생활습관과 난임 치료의 관계에 대한 연구(Goldman et al., 2019; Nassan et al., 2018; Sacha et al., 2018; Setti et al., 2018), 보조생식술에서 연령의 영향에 대한 연구(Pinheiro et al., 2019; Somigliana et al., 2020), 난임 치료를 받는 부부들의 친밀감 및 삶의 질에 관한 연구(Goker et al., 2018; Luk & Loke, 2019), 보조생식술의 다양한 임상 결과에 영향을 미치는 연령, 호르몬의 영향 등에 대한 연구가 수행되어 왔다(Behre, 2019; Hu et al., 2018; Pinheiro et al., 2019; Somigliana et al., 2020; Yovich et al., 2019). 하지만 코로나 팬데믹이라는 사회환경적 맥락의 전환을 겪으면서, 난임 치료 참여자 대상의 연구 경향에는 어떤 변화가 있었는지를 파악할 필요가 있다.
이에 이 연구에서는 코로나 팬데믹 기간이 난임 치료 참여자에게 어떠한 영향을 미치는지에 대한 선행연구를 포괄적으로 검토하여 최근 발표된 문헌의 주제를 종합하고, 포스트 팬데믹 전환기에 우리나라의 난임 치료 방향에 대한 함의를 도출하고자 한다. 구체적인 목적은 코로나 팬데믹 시대의 난임 치료 참여자 대상 연구의 주요 주제를 파악하고, 각 주제별 종속변수에 대한 측정한 도구와 분석 결과의 공통점과 차이점을 분석하며, 기존에 국내에 발표된 연구와 비교를 통해, 난임 치료 참여자에게 미친 코로나 팬데믹의 영향에 관한 최신 연구의 단면(snapshot)을 파악하는 것이다. 이를 통해 국제적으로 연구가 활발히 진행되는 테마의 동향을 분석하여 국내 연구의 방향성을 제시하며 포스트 팬데믹 전환기에 주는 시사점을 도출하고자 한다.

대상 및 방법

이 연구는 코로나 팬데믹이 여성의 난임 치료에 어떤 영향을 미쳤는지에 대한 현존하는 연구의 동향을 파악하기 위하여 서술적 설계로 진행된 문헌고찰 연구로서, 그동안 수행된 연구의 주제 및 연구 결과를 확인하며, 연구가 충분히 수행되지 않은 영역을 확인하며 향후 연구의 방향을 제시하고자 스코핑 리뷰(scoping review, 주제범위 문헌고찰)를 사용했다. 이 방법은 연구영역을 뒷받침하는 주된 개념, 자료원, 가용한 근거의 유형을 빠르게 매핑하기 위한 목적으로 수행되며, 체계적 문헌고찰의 잠재적인 주제범위를 조사하기 위한 구조화된 접근법이다(Arksey & O'Malley, 2005). 특히 빠르게 변화하는 근거합성에 유용한 방식으로, 우선순위를 설정하기 위한 기초자료로 활용된다(Daudt et al., 2013; Munn et al., 2018; Seo & Kim, 2018). Arksey와 O'Malley (2005)는 스코핑 리뷰 연구 절차 6단계로 ‘(1) 연구 질문 도출, (2) 관련문헌 조사, (3) 문헌 선정, (4) 자료 기입, (5) 결과의 수집, 요약, 보고, (6) 자문(선택적 단계)’를 제시했으며, 이 연구에서는 1-5단계를 적용하여 문헌을 선정했다.

[1단계] 연구 질문 도출(identifying the research question)

이 연구에서는 “COVID-19이 난임 치료에 어떤 영향을 미쳤는가?”에 대한 주제로 연구 질문으로 설정하였다.

[2단계] 연구검색(identifying relevant studies)

(1) 국내 문헌: 코로나 난임 치료

국내학술검색엔진은 학술연구정보서비스(RISS), 의과학연구정보센터(KMbase), 대한의학학술지(KoreaMed)에서 ‘코로나’, ‘난임 치료’를 키워드로 검색한 결과, 관련 논문은 없는 것으로 확인됐다.

(2) 국외문헌

문헌 검색 기간은 2022년 7월 28일, 8월 19일이었고, 문헌 발행 기간에는 제한을 두지 않고 발표된 모든 문헌을 연구 대상으로 했다. 그럼에도 COVID-19이라는 주제어 때문에 2020부터 2022년까지 발행된 논문이 검색되었다. 국외학술검색엔진은 PubMed 및 코크란 라이브러리(The Cochrane Library)에서 ‘COVID-19’, ‘infertility treatment’가 연구제목(Title) 또는 초록(Abstract)에 포함된 연구 동향을 탐색했다.

[3단계] 문헌 선정(study selection)

문헌 선정은 EQUATOR (Enhancing the QUAlity and Trans-parency Of health Research) Network의 연구보고지침에서 제안한 스코핑 리뷰 흐름도와 점검표(PRISMA-ScR Checklist)를 참고했다(Tricco et al., 2018). PubMed 및 코크란라이브러리 기반의 온라인 학술데이터 검색을 통해 ‘COVID-19’, ‘infertility treatment’가 연구제목 또는 초록에 포함된 29편의 논문이 검색되었다. 이 중에 중복되는 문헌 1편을 제외하고, 제목과 초록을 검토하여 연구 주제에 맞지 않는 문헌 12편을 배제하였다. 16편으로 추려진 논문에 전문을 읽고 확인하였으며 배제 기준(연구 대상자, 연구 방법, 주제)을 확인한 후 산부인과 클리닉 웹사이트 분석 논문 1편, 남성 불임에 관한 사례보고 1편, 난임 치료중재 임상 프로토콜 개발 연구 1편을 제외하였고, 최종적으로 13편의 분석 대상 문헌을 선정했다(Fig. 1).

[4단계] 자료 기입(charting the data)

13개의 문헌은 Microsoft Excel을 사용하여 자료 기입 서식에 따라 저자, 출판 연도, 연구 수행 국가, 연구 대상자, 연구 목적, 연구 방법, 주요 결과 등을 포함하여 기록하였다. 2명의 연구자가 연구기록 양식에 논문의 내용을 기입하고, 3명의 연구자가 정기적인 모임을 통해 연구 내용을 확인하고 연구 주제에 따른 분류 및 연구 결과에 대해 의견을 교환하여 최종적인 내용을 정리했다.

[5단계] 결과 수집, 요약, 보고(collating, summarizing, and reporting the results)

스코핑 리뷰에서는 분석한 자료를 개괄적으로 표현하여 자료가 얼마나 잘 설명될 수 있는지가 중시된다(Arksey와 O’ Malley, 2005). 이 연구에서는 PRISMA-ScR의 스코핑 리뷰 방법을 참고하여 연구 결과를 분석 후 합의된 결론을 도출하였으며, 고찰된 결과를 요약한 표 및 그림으로 제시하였다(Tricco et al., 2018).

결 과

1. 대상 문헌의 일반적 특성

최종 13편의 문헌에 대한 일반적 특성은 Table 1과 같다. 출판 연도를 제한하지 않았지만, COVID-19와 난임 치료와 관련 연구는 모두 2020년 이후 발표되어, 2020년 2편(Braga et al., 2020; Vaughan et al., 2020), 2021년 7편(Cao et al., 2021; Cirillo et al., 2021; Ilska et al., 2021; Lawson et al., 2021; Lin et al., 2021; Mitrović et al., 2021; Seifer et al., 2021), 2022년 4편(Aizer et al., 2022; Avraham et al., 2022; Pretzel et al., 2022; Vastis et al., 2022)이었고 모두 국외 저널에 발표된 연구였다.

연구 수행 지역을 살펴보면 유럽에서 3편[이탈리아(Cirillo et al., 2021), 폴란드(Ilska et al., 2021), 세르비아(Mitrović et al., 2021)], 중동에서 2편[이스라엘 (Aizer et al., 2022; Avraham et al., 2022)]의 논문이 연구되었고, 아시아에서 2편[중국(Cao et al., 2021), 대만(Lin et al., 2021)]의 논문이 발간되었으며 미국 및 캐나다 5편[미국(Lawson et al., 2021; Pretzel et al., 2022; Seifer et al., 2021; Vaughan et al., 2020), 캐나다(Vastis et al., 2022)]에서 최다 논문을 발표하였고, 브라질에서 발표한 논문이 1편(Braga et al., 2020)이었다.
발표된 연구의 저널을 학술분야에 따라 분류하면 생식의학(reproductive medicine, fertility & sterility)분야에서 10편(Aizer et al., 2022; Avraham et al., 2022; Braga et al., 2020; Cirillo et al., 2021; Ilska et al., 2021; Lawson et al., 2021; Pretzel et al., 2022; Seifer et al., 2021; Vastis et al., 2022; Vaughan et al., 2020)으로 가장 많고, 정신의학 및 심리학(psychiatry, psychology)분야에서 2편(Cao et al., 2021; Mitrović et al., 2021), 보완의학(COMPLEMENTAry medicine)분야는 1편(Lin et al., 2021)이었다.

2. 연구주제, 자료원 및 연구 참여자 특성

각 논문의 주제어와 검토한 본문 내용을 바탕으로 연구 주제는 크게 3가지로 분류되었다(Table 2). 첫째, 코로나 팬데믹으로 인한 난임 치료 참여자에 대한 심리상태를 분석한 연구로서, 코로나 팬데믹으로 인한 스트레스와 불안 지수를 측정한 연구 논문이 7편(Cao et al., 2021; Cirillo et al., 2021; Ilska et al., 2021; Lawson et al., 2021; Mitrović et al., 2021; Seifer et al., 2021; Vaughan et al., 2020)이었다. 여기에는 회복탄력성을 매개변수로 활용하여 코로나 팬데믹에 대체 능력에 따라 스트레스 정도가 달라진다는 연구도 포함되었다(Seifer et al., 2021). 둘째, 코로나 팬데믹 시기의 난임 치료 참여자의 대한 임신 성공률 변화에 관한 연구이다. 임신 성공률 여부에 대한 연구는 총 3편(Aizer et al., 2022; Avraham et al., 2022; Vastis et al., 2022)으로 모두 임신 성공률에 대해 부정적인 영향을 끼치지 않았다는 결과가 도출되었다. 셋째, 코로나 팬데믹으로 인한 임신 계획과 난임 치료 참여 정도의 변화를 연구한 논문은 총 3편(Braga et al., 2020; Lin et al., 2021; Pretzel et al., 2022)이었으며, 임신 계획 변화(Braga et al., 2020), 치료인식(Pretzel et al., 2022) 및 의료이용의 변화(Lin et al., 2021)로 구성되었다.

연구 자료원을 살펴보면, 코로나 팬데믹으로 인한 스트레스, 우울에 관한 연구(7편), 임신 계획이나 치료 참여에 대한 연구(2편)는 설문조사를 통한 데이터 수집 및 단면연구 방식을 활용했고, 임신 성공률을 결과 변수로 사용한 연구(3편)는 병원 데이터 기반의 후향적 코호트 방식을 활용했다. 코로나 팬데믹 기간 동안 난임 치료에 대한 전통보완의학 서비스 이용에 대한 연구 1편만 병원정보시스템의 청구데이터를 분석한 연구였다.
연구 참여자는 난임 치료를 받고 있는 여성을 대상으로 한 경우가 5편(Avraham et al., 2022; Lawson et al., 2021; Seifer et al., 2021; Vastis et al., 2022; Vaughan et al., 2020), 비난임 여성인 대조군을 포함해서 가임기 여성 전체를 대상으로 한 경우가 6편(Aizer et al., 2022; Braga et al., 2020; Cao et al., 2021; Cirillo et al., 2021; Ilska et al., 2021; Mitrović et al., 2021), 자궁내막증 진단을 받은 여성을 대상으로 한 경우가 1편(Pretzel et al., 2022) 이었다. Lin 등(2021)은 전자의무기록(electronic medical record, EMR) 자료를 활용해서 코로나 팬데믹 전후 보완의학 중 난임 치료 환자 수를 비교하였으므로 대상자에 제한은 없었다. 난임 여성을 대상으로 하는 경우에는 평균 연령이 35세 였고, 전체 여성을 대상으로 하는 경우에는 대부분 가임기 여성을 대상으로 해서 만 18-49세를 대상으로 하고 있었다. 다만 Cao 등(2021)의 연구는 18-59세 여성을 대상으로 하여 난임 여성이 겪는 어려움을 비난임 여성과 비교했다.
대부분 난임 병원의 환자를 대상으로 설문조사를 하거나 후향적 코호트 분석을 수행했다. 연구 대상자 수는 30명 미만인 경우 1편(Pretzel et al., 2022), 100-299명인 경우 3편(Cirillo et al., 2021; Mitrović et al., 2021; Seifer et al., 2021), 300-999명인 경우 5편(Aizer et al., 2022; Avraham et al., 2022; Cao et al., 2021; Lawson et al., 2021), 1,000명 이상인 경우 3편(Ilska et al., 2021; Vastis et al., 2022; Vaughan et al., 2020), 병원정보시스템 기반으로 청구자료가 모두 포함되어 표본수가 약 200만명에 가까운 경우가 1편(Lin et al., 2021)이었다.

3. 코로나 팬데믹이 난임 치료 참여자에 미치는 영향에 대한 연구 결과 종합

코로나 팬데믹의 영향을 측정한 세가지 측면을 난임 치료 참여자의 스트레스와 불안 및 우울, 임신 성공률의 변화, 난임 치료를 통한 임신 계획 및 난임 치료에 대한 참여 양상으로 구분하여 문헌들의 연구 결과를 종합하면 다음과 같다(Fig. 2). 첫째, 코로나 팬데믹 기간이 난임 치료 참여자의 스트레스와 불안 지수에 어떠한 영향을 미쳤는지에 대한 연구 결과를 종합해 보면, 코로나 팬데믹으로 인해 난임 치료에 대한 스트레스 및 불안지수가 악화되었다는 연구가 5편(Cao et al., 2021; Cirillo et al., 2021; Ilska et al., 2021; Lawson et al., 2021; Vaughan et al., 2020) 심리상태에 변화가 없는 연구가 1편(Mitrović et al., 2021) 긍정적인 변화가 있다는 연구가 1편(Seifer et al., 2021)이었다. 코로나 팬데믹 기간에 난임 치료 참여자의 심리상태에 긍정적인 변화가 있다는 연구의 경우, 난임 치료를 일시적으로 중단하더라도 대처 방법을 가지고 있는 여성일수록 불안과 스트레스 척도가 낮다는 결과를 보여주었다(Seifer et al., 2021).

둘째, 코로나 팬데믹과 난임 치료 참여자의 임신 성공률 변화에 관한 연구 결과를 살펴보면, 임신 성공률 여부에 대한 연구는 총 3편(Aizer et al., 2022; Avraham et al., 2022; Vastis et al., 2022)으로 모두 임신 성공률에 대해 부정적인 영향을 끼치지 않았다는 결과가 도출되었다. 3편 중에서 2편(Aizer et al., 2022; Avraham et al., 2022)은 COVID-19 백신 접종 여부를 중점으로 그룹 간의 임신 성공률을 비교한 것이다. 2편의 논문 모두 다 백신은 BNT162b2(화이자-바이오엔텍)을 사용하였다. Avraham 등(2022)의 논문은 백신 접종자와 비접종자 간의 난모세포수와 임상 임신 성공률의 차이를 비교하였다. Aizer 등(2022)의 논문의 경우, COVID-19 백신을 접종하거나 감염에서 회복된 면역그룹과 COVID-19 백신을 접종하지 않거나 감염된 경험이 없는 비면역그룹(2021년), COVID-19 대유행 전에 치료를 받은 비면역그룹(2019년)의 3그룹의 냉동 배아 이식률을 비교하였다. 나머지 1편(Vastis et al., 2022)은 난임 치료 중단에 따른 임신 성공률 영향을 평가하였으며, 이 결과도 마찬가지로 임신 성공률에 변화가 없었다.
셋째, 코로나 팬데믹으로 인한 난임 치료를 통한 임신 계획 및 치료에 대한 참여 양상의 변화를 측정한 연구 결과를 종합하면 다음과 같다. 임신 계획의 경우, 보조생식 시술을 받는 실험군과 시술을 받고 있는 대조군 모두 임신 계획을 연기한다는 설문 결과가 도출되었다(Braga et al., 2020). 치료 인식의 경우 난임 여성 중에서 자궁내막증을 가지고 있는 집단의 자궁내막증 치료 여부 인식의 변화는 COVID-19의 영향이 없었다(Pretzel et al., 2022). 대만의 EMR 데이터를 분석한 연구에서는 코로나 팬데믹 기간인 2020년에 전통 보완의학 내에서도 산부인과의 이용이 약 40%가 감소하고, 난임 치료를 받은 경우도 기간에 따라 5%-52%까지 2019년에 비해 유의미하게 감소한 것으로 나타났다(Lin et al., 2021).

고 찰

이 연구는 코로나 팬데믹이 난임 치료 참여자에게 어떠한 영향을 미치는지에 대한 연구동향을 살펴보기 위해, 최종 선정된 13편의 문헌에 대한 연구 주제와 연구 결과를 고찰했다. 문헌고찰 대상 논문에서 가장 높은 비중을 차지하는 것은 난임 치료를 받은 여성들의 스트레스 및 불안 등의 심리적 변화에 관한 주제(7편, 53.8%)였고, 임신 성공률 변화에 관한 연구(3편, 23.1%), 임신 계획 및 의료 이용에서의 변화(3편, 23.1%)에 대한 연구가 있었다. 이러한 선행연구를 종합하면 코로나 팬데믹은 임신 성공률과 같은 임상적인 결과에는 유의미하게 영향을 미치지 않았지만, 우울과 스트레스에는 부정적으로, 임신 계획 및 의료 이용은 낮추는 방향으로 영향을 미치는 것으로 보고되고 있었다.
코로나 팬데믹 이전에도 난임 치료 참여자의 심리상태에 관한 연구가 다수 이루어져 왔다. 주로 보조생식술을 받는 여성의 스트레스 수준, 우울과 불안 수준을 측정하고, 이러한 심리적 요인과 삶의 질의 관련성을 측정하거나, 우울감에 영향을 미치는 배우자와의 관계 및 친밀성 등에 관한 연구였다(Cheng et al., 2018; Freeman et al., 2018; Jung & Kim, 2017; Kim et al., 2016; Kim, 2018; Kim et al., 2021; Massarotti et al., 2019; Nicoloro-SantaBarbara et al., 2018). 코로나 팬데믹 이전에는 임신성공률에 영향을 미치는 난임 치료를 받는 환자의 특성 파악에 중점을 둔 연구가 수행되었는데 특히 보조생식술의 성공에 영향을 미치는 요인에는 호르몬, 라이프스타일, 사회경제적 지위 등 다양한 예측요인이 밝혀졌다(Behre, 2019; Goldman et al., 2019; Hu et al., 2018; Nassan et al., 2018; Pinheiro et al., 2019; Sacha et al., 2018; Setti et al., 2018; Somigliana et al., 2020; Yovich et al., 2019). 그러나 코로나 팬데믹 기간에는 환경적 변화에 대한 관심이 개인의 라이프스타일에 대한 관심보다 커지면서 개인의 생활습관 개선 요인을 찾는 연구는 찾기 어려웠다. 마찬가지로, 코로나 팬데믹 이전 자료를 활용하여 사회경제적 수준에 따른 난임 치료에 대한 접근성과 비용분석에 관심이 있었으나(Kim & Hwang, 2016; Moon & Jeon, 2022), 코로나 팬데믹으로 인하여 난임 치료 참여 의사에 변화가 있었는지에 대한 조사연구가 수행되었다. 또한 코로나 팬데믹 이전에는 난임 치료에서 전통보완의학의 효과성을 분석한 연구가 다수 있었으나(Hwang et al., 2019; Kim et al., 2018), 코로나 팬데믹 기간에는 전염병으로 인한 산부인과 진료 및 난임에 대한 보완의학 수요가 감소했다는 연구가 있었다(Lin et al., 2021).
코로나 팬데믹 이후 발표된 13편의 연구를 주제별로 고찰하면 다음과 같다. 우선 코로나 팬데믹 기간에 난임 여성의 스트레스와 관련된 연구를 살펴보면(Ilska et al., 2021; Vaughan et al., 2020), 미국에서 2202명의 난임 여성을 대상으로 한 설문조사에서, 코로나 팬데믹 중에도 불구하고 난임은 여전히 가장 큰 스트레스 요인이며(Vaughan et al., 2020), 폴란드에서 가임기 여성 1,148명을 대상으로 한 설문조사에서는 난임 환자의 팬데믹 관련 임신 스트레스가 난임 치료를 받았던 여성, 초산임산부, 임신 중 고위험 환자에서 높은 것으로 나타났다(Ilska et al., 2021). 또한 코로나 팬데믹은 난임 여성의 불안 및 우울과 관련 있었다. Lawson 등(2021)은 미국에서 코로나19로 인해 지연된 난임 치료에 대한 심리적 고통을 평가했는데, 응답자 대다수가 치료지연에 괴로워했고, 치료지연으로 인한 우울증과 불안을 경험한다고 밝혔다. 코로나 팬데믹으로 인해 치료를 일시 중지한 난임 환자의 심리적 경험과 대처 전략을 평가한 연구에서는 변화에 대한 대처기술이 중요한 요소임을 보여주었다. 일상생활하기, 정기적으로 밖에 나가기, 운동하기, 전화, 소셜미디어 등의 사회적 관계 유지를 통한 회복탄력성 점수가 높을수록 불안감은 감소하는 것으로 나타났고, 반면 회복력이 낮은 사람은 이전에 난임 치료로 경구 약물 복용한 경우 상태불안 정도가 높았다(Seifer et al., 2021). Mitrović 등(2021)은 세르비아에서 코로나 팬데믹 상황에서 체외수정 진행이 지연된 여성의 불확실성에 대한 인내력을 조사했는데, 여성들의 회피 전략은 난임 치료 지연과 일반적인 고통 사이에서 부분 매개자 역할을 하는 것으로 분석되었다. 한편, 물리적 격리상황은 난임 여성에게 더욱 불안을 증폭시켰는데 중국의 경우, 장기 난임 치료 환자나 체외수정을 해야 하는 환자는 불안함이 크게 나타났으며(Cao et al., 2021), 이와 유사하게 Cirillo 등(2021)은 이탈리아에서 난임 치료를 계획하는 여성의 생활 방식과 감정 상태에 대한 코로나19 봉쇄의 영향을 평가했는데, 검역으로 인한 일상생활의 제한은 라이프스타일과 심리적 행동에 부정적 영향을 미쳤고, 심혈관 질환의 부담 증가 및 심리적 행동에 부정적인 영향을 미치는 것으로 나타났다.
난임 치료를 받는 환자의 스트레스, 불안, 우울을 측정하기 위해 전문적인 심리 측정 도구를 사용하고 있었다. 선행연구에서 활용된 측정도구를 살펴보면, 팬데믹 관련 임신스트레스 척도(Pandemic-Related Pregnancy Stress Scale) (Ilska et al., 2021), 상태불안척도(State Trait Anxiety Inventory) (Cao et al., 2021; Seifer et al., 2021), 범불안장애 7항목 척도(Generalized Anxiety Disorder-7 scale) (Lawson et al., 2021), 불확실성에 대한 인내력 척도(Intolerance of Uncertainty Scale) (Mitrović et al., 2021), 대처전략검사(Coping Strategy Indicator) (Seifer et al., 2021), 우울, 불안 및 스트레스 척도(Depression Anxiety Stress Scale-21, DASS-21) (Mitrović et al., 2021) 등이 있었다. 국내에서도 그동안 난임 환자의 스트레스, 우울과 관련된 연구가 수행되어 왔으나 코로나 팬데믹이라는 환경적 요소는 고려되지 못한 상황이다.
Mitrović 등(2021)의 연구에서 난임 환자의 우울, 불안, 스트레스를 측정하기 위해 우울, 불안 및 스트레스 척도(DASS-21)를 사용했는데, 국내 난임 환자를 대상으로 한 연구에서도 심리검사를 위해 한국판 우울 불안 스트레스 척도-21 (Korean version of Depression Anxiety Stress Scale, K-DASS-21)을 사용해 왔다(Kim et al., 2020; Lee & Kwon, 2020; Lee et al., 2019;). 별도로 난임으로 인한 스트레스를 측정하기 위해서는 Newton 등(1999)에 의해 개발된 난임 문제 목록(Fertility Problem Inventory)을 Kim과 Shin (2013)이 번안한 도구가 가장 많이 사용되고 있으며(Kim, 2016; Kim et al., 2021; Lee & Park, 2019; Song & Jee, 2021), 난임 스트레스 척도를 활용하기도 했다(Kim et al., 2020). 난임 환자의 우울은 기존에 우울 측정에 자주 쓰이고 있는 도구에는 한국판 역학연구 우울척도 개정판(Korean version of Center for Epidemiologic Studies Depression Scale-Revised)이 있고(Kim, 2016; Song & Jee, 2021), 벡 우울척도(Beck Depression Inventory-II)가 활용되기도 했으며(Jung & Kim, 2017), 한글판 우울 선별도구(Patient Health Questionnaires-9)가 쓰여왔다(Kim et al., 2020). 다만 코로나 팬데믹과 같은 위기상황을 반영한 도구는 발표되지 않았는데, 포스트 팬데믹 전환기이자 난임 환자 수가 지속적으로 증가하는 상황에서 이들의 불안 정도나 스트레스, 우울 등을 조사하기 위한 타당성 있는 측정도구의 개발 및 기초연구가 필요함을 보여준다.
다음으로, 코로나 팬데믹 시기의 임신성공률 변화 관련 연구를 살펴보면, 이 주제에 관한 연구 3편은 모두 후향적 코호트 방식을 사용했다. 난임 치료를 통한 임신 성공률에는 다양한 요인이 작용하고 하나의 원인을 특정하기 어렵기 때문에, 선행연구에서는 주로 백신이 체외수정의 결과에 미치는 영향(Aizer et al., 2022; Avraham et al., 2022), 코로나로 인한 난임 치료 프로토콜의 조정이 난임 치료 결과에 미치는 영향을 비교 분석했다(Vastis et al., 2022). 이스라엘에서 수행된 Avraham 등(2022)의 연구에서는 2021년 4월까지 체외수정을 받은 환자 중에 백신 예방접종을 받은 환자 200명과 연령이 일치하는 미접종 여성 200명의 평균 난모세포 수와 임상 임신율을 비교했는데, 코로나19 메신저 리보핵산(mRNA) 백신 접종은 체외수정 치료에서 난소 반응이나 임신율에 영향을 미치지 않는 것으로 나타났다. 또한 같은 나라에서 수행된 Aizer 등(2022)의 연구는 2021년 1월-8월 코로나 바이러스 감염증에 의한 감염 또는 메신저 리보핵산(mRNA) 백신접종을 통해 면역을 획득한 그룹 141명과 비면역 그룹 194명 사이에서 동결보전 배아이식(frozen-thawed embryo transfer, FET)에 미치는 영향을 분석했는데, 감염 또는 백신접종은 환자의 후속 동결보전 배아이식(FET) 주기에 영향을 미치지 않았다. 한편 캐나다에서 수행된 Vastis 등(2022)의 연구에서는 코로나 팬데믹 기간 동안 난임 치료를 받은 환자와 이전 환자의 임신율을 비교했는데, 코로나 팬데믹 기간에 프로토콜이 조정된 것을 감안하더라도, 팬데믹 이전 그룹[인공수정(intrauterine inseminatio, IUI) 617명, 체외수정(in vitro fertilization, IVF) 226명, 동결보전 배아이식(FET) 260명]과 팬데믹 기간 그룹[인공수정(IUI) 634명, 체외수정(IVF) 224명, 동결보전 배아이식(FET) 318명] 사이에 난임 치료 결과인 임신율은 유의미한 차이가 발생하지 않는 것으로 분석되었다.
국내에서 수행된 난임 치료를 통한 임신성공에 영향을 미치는 요인에 관한 분석을 살펴보면, 여성의 연령이 공통적으로 중요한 인자로서 Jang 등(2015)에서 원인불명 난임부부의 임신 성공에 영향을 미치는 요인을 살펴본 결과, 시술유형 중 경구제와 주사제를 병용한 경우, 여성의 연령이 젊을 때에, 인공수정을 통한 임신 성공이 높아졌으며, Kim & Hwang (2016)의 연구에서도 보조생식술의 성공은 여성의 연령, 난임 기간, 출산 자녀 수와 관련 있고 특히 여성의 연령이 임신에 주요 요인으로 분석되었다. 반면 체외수정시술을 받는 난임 여성에서 건강증진형 운동 활동은 임신율에 유의미한 영향을 미치지 않는 것으로 나타났다(An et al., 2021). 국내에서는 코로나백신이 임산부에 미치는 영향에 대한 연구가 수행되어 왔는데, 난임을 주제로 한 연구는 찾기 어려웠다. 예컨대 Lee 등(2022)의 연구에 의하면, COVID-19 확진 진단을 받고 입원한 총 184명의 산모 중에 COVID-19 감염이 확인된 임산부에서 심각한 임상 증상을 발생할 가능성이 적었으며, COVID-19 감염으로 인해 사망한 산모는 없었으나 델타 변이 대유행 이후 산모의 합병증은 더 심각하였고, 임신 중 혹은 분만 중 COVID-19의 수직 감염은 없어 수직 전파의 증거는 없는 것으로 보고되고 있다.
마지막으로 코로나 팬데믹으로 인한 난임 치료 계획 및 치료양상에 관한 연구를 살펴보면, 코로나 팬데믹으로 인한 치료지연, 백신 등은 임신 성공률의 변화에 영향이 없었음에도 일부 여성들은 치료계획을 미루는 경향이 나타났다. Braga 등(2020)은 브라질에서 보조생식치료 그룹 및 체외수정에 관심이 있는 여성을 대상으로 코로나19 대유행에 대한 인식과 모성계획에 대한 정보를 수집했는데, 2020년 4-5월 모성계획에 대해 조사한 결과, 코로나 팬데믹 기간에 일부는 출산 계획을 연기했지만 대부분은 임신 계획 자체를 포기하지는 않은 것으로 나타났으며, 코로나 질병에 대한 두려움 외에도 경제적 부담이 모성계획 지연에 가장 방해가 되는 요인으로 조사되었다. 또한 Pretzel 등(2022)은 미국에서 자궁내막증 진단을 받은 여성 중에서 체외수정 전에 추가로 자궁내막증 치료에 대한 참여의사를 조사한 결과, 체외수정을 계획한 여성은 코로나 팬데믹으로 인해 자궁내막증 치료의사에 영향을 받지 않았고, 여전히 치료 후 체외수정을 준비하고 있는 것으로 나타났다. 다만 대만에서 전통보완의학으로 서비스 유형을 한정했을 때, 대만의 농촌지역에서는 코로나 팬데믹 동안 보완의학 전문 분야 중 난임 치료는 다른 진료과에 비해 환자 수가 가장 크게 감소한 것으로 나타났다(Lin et al., 2021).
국내에서는 코로나 팬데믹으로 인한 난임시술 치료 계획의 변화나 코로나 팬데믹 이후 난임 환자 증감에 대한 연구를 찾기 어려웠다. 다만 난임시술의 경험에 관한 질적연구를 통해 짧은 진료시간, 전문적인 의학정보의 어려움, 심리정서적 배려의 부재, 경제적 부담감 등 환자중심 보다는 의료 중심적인 진료 환경이 난임시술에 대한 접근성을 저해한다고 했다(Jeon & Kim, 2022). 한편, 인과관계에 대한 연구는 아니지만 건강보험심사평가원 통계에 따르면 난임 치료 진료를 받은 환자 수는 2020년 대비 2021년 오히려 증가하는 추세인데(Health Insurance Review and Assessment Service, 2023), 재택근무가 많아지면서 오히려 시간적으로 여유가 있는 고소득 여성들이 난임 치료를 적극적으로 받았을 수 있다.
난임 치료는 환자의 연령이 성공에 중요한 영향 요인이기 때문에 시기가 굉장히 중요한 치료로 알려져 있다(Jang et al., 2015; Kim & Hwang, 2016). 코로나 팬데믹 등을 이유로 그동안 백신접종 및 코로나 감염의 영향에 대한 불확실성 속에서 많은 여성들이 불안, 스트레스, 치료지연을 경험해 온 것으로 파악된다. 이러한 상황을 방지하기 위해 위기상황 속에서 난임 여성 대상의 표준화된 정보전달과 교육, 의료진과의 충분한 의사소통이 필요함을 보여준다. 주요 선진국에서는 정부 주도의 공적 의료서비스 내에서 난임 상담사업을 수행하고 있으며(Choi & Lee, 2020), 우리나라에서도 모자보건법을 근거로 2018년 국립중앙의료원에 중앙난임 · 우울증상담센터 및 지역별 권역센터가 개소하여 난임 환자와 임산부 등 산부인과 환자들의 심리적 어려움을 지원하는 사업을 운영하고 있다(Kim et al., 2020). 그럼에도 난임 환자들이 코로나 등 위기상황에서 일차적으로 정보를 얻을 수 있는 곳은 블로그, 카페 등의 인터넷 상의 정보가 주를 이루는 상황이기도 하다. 이 연구의 문헌고찰을 통해, 난임 환자들이 근거기반의 공신력 있는 정보를 접할 수 있도록 온라인, 앱 기반의 다양한 정보전달 채널을 활용할 것이 필요하며, 이를 통해 불필요한 불안, 스트레스, 치료의 지연을 방지하기 위한 노력이 요구됨을 시사한다.
이 연구의 제한점은 다음과 같다. 스코핑 리뷰 연구 방법의 한계상 근거문헌의 질 평가를 거치지 않아 잠재적 비뚤림의 위험이 있으며(Munn et al., 2018), 학술지에 게재된 논문만 대상으로 하고 학위논문, 보고서, 학술대회 초록은 연구 대상에서 제외되어 대상 문헌 수가 많지 않았다. 특히 코로나 팬데믹의 영향을 난임 치료에 한정하여 분석한 국내문헌은 발표되지 않아서 포함할 수 없었다. 그럼에도 이 연구는 최근 우리 사회와 일상에 가장 큰 변화를 가져온 코로나 팬데믹이 난임 치료를 받는 환자들에게 어떤 영향을 미쳤는지에 대해 최신 동향을 살펴보고, 주된 주제와 결과를 통해 심리적 특성, 난임 치료의 임상적 결과 특성, 난임 치료에 대한 의향 및 이용의 특성에 대한 결과를 통해 시의성 있는 근거합성을 시도했다는데 의의가 있다.

결 론

이 연구에서는 스코핑 리뷰를 통한 문헌고찰 방법을 사용하여 코로나 팬데믹이 난임 치료 참여자에게 어떤 영향을 미쳤는지에 관한 연구 동향을 분석하여 다음과 같은 결과를 얻었다. 우선, 2020년 이후 발표된 코로나 팬데믹이 난임 치료에 미친 영향에 관한 13편논문의 연구 주제는 코로나 팬데믹으로 인한 난임 치료 참여자의 스트레스 및 불안, 우울 수준을 측정한 연구(7편), 임신 성공률의 변화(3편), 임신 계획 및 난임 치료 참여 의향의 변화(3편)라는 3가지로 구분할 수 있었다. 종합적으로 임신율에 부정적인 영향은 없었지만, 이 기간 동안 스트레스와 불안이 높아짐에 따라 심리적 어려움을 겪은 참여자들의 상황이 보고되었으며 임신 계획을 연기하기도 했지만 난임 치료에 대한 의지는 변함이 없는 것으로 분석되었다. 이러한 연구 경향을 통해, 우리나라에서도 코로나 팬데믹 시기 및 포스트 코로나 시기의 심리상태 및 임상 결과를 측정하고, 난임 치료 참여자에게 근거 기반의 정보제공을 할 수 있는 구조가 갖추어질 필요가 있음을 보여준다. 이 연구는 코로나 팬데믹 전환기를 맞이하는 시점에서 코로나 팬데믹과 난임 치료 참여자에 관한 연구의 동향을 제시하고 국내 난임 치료 참여자를 대상으로 하는 근거 마련의 필요성을 제시했다는 데에 의의가 있다.
