아동 간호에서 간호사-부모 파트너십: 주제 범위 문헌고찰

ABSTRACT

Purpose

This study aimed to identify the factors associated with nurse-parent partnerships in pediatric settings and examine the elements involved in their formation based on national and international literature. The findings aim to inform future research directions.

Methods

A scoping review was conducted in February 2025 following established methodological frameworks. Studies published in English or Korean were searched across electronic databases, including RISS, KMbase, KoreaMed, NDSL, DBpia, PubMed, CINAHL, MEDLINE, and Web of Science. In total, 912 records were identified, and 35 were included in the final analysis.

Results

Most studies were descriptive in design and were published after 2016, with a predominance of research conducted in South Korea. Nurse-related factors, including self-efficacy, emotional labor, emotional intelligence, communication competence, and clinical decision-making, were associated with nurse-parent partnerships. Parent-related factors were relatively limited. Researchers reported significant correlations between several variables, including nursing professionalism, intention to stay, and fatigue, and nurse-parent partnerships. Outcomes associated with nurse-parent partnerships included quality of care, job satisfaction, job stress, and nursing competence. Parent-related outcomes included anxiety, parental satisfaction, self-efficacy, and readiness for discharge. The major elements of partnership formation included respect, communication, information sharing, emotional support, individualized care, and parental participation.

Conclusion

Future research should expand experimental studies to promote nurse-parent partnerships. Researchers should include parents in future studies given their significance in pediatric settings. Additional research is needed to identify antecedent and outcome factors by incorporating a broader range of related variables.

서 론

아동 간호에서 주요 철학으로 여겨지는 가족 중심 돌봄(family centered care, FCC)은 의료진 중심으로 운영되던 전통적인 의료 모델에 대해 문제 제기와 함께 등장하였다(Almalki et al., 2025; Andretta et al., 2025). 또한, 아동이 어머니와 분리될 경우 아동에게 정서적, 심리적, 발달적 측면으로 부정적인 결과가 발생한다는 연구가 보고되기 시작하면서 변화에 대한 인식이 확산되었다(Harrison, 2010). 이후 부모가 아동의 치료와 회복 과정에 중요한 역할을 한다는 인식이 증가하면서 가족 중심 돌봄이 대두되었다(Almalki et al., 2025; Smith et al., 2015). 또한, 의료 서비스에서 대상자의 관점과 가치가 중요하게 인식되면서 입원 아동 가족의 요구를 충족하고 만족도를 향상시키는 것에 대한 중요성이 더욱 강조되었다(Jung & Tak, 2017). FCC는 파트너십, 의사소통, 존중, 공감의 주요 개념으로 설명할 수 있으며(Seniwati et al., 2023), 이 가운데 파트너십은 가족과 의료진이 함께 돌봄을 제공하기 위한 중요한 개념으로 제시된다(Im & Park, 2024).
파트너십은 일반적으로 두 명 이상의 개인이 함께 일하며 파트너 또는 관계를 맺는 것을 의미한다(Oxford University Press, 2023). 의료 분야에서 파트너십은 존중을 바탕으로 상호 협의를 통해 함께 일하는 방식으로, 이는 공감과 지지를 기반으로 대상자에게 선택권을 부여하고, 치료에 참여하도록 하며, 의료진과 동등한 관계를 형성하게 한다(Bidmead & Cowley, 2005). 또한 파트너십이 향상될수록 건강 돌봄의 효과와 질을 향상시키고(Lee & Choi, 2024), 대상자의 만족도를 높이는 것으로 보고되었다(Andretta et al., 2025). 아동 간호에서는 대상자의 의미가 부모와 가족으로 확대되어 간호사와 아동의 부모가 공동의 목표를 가지고 아동에게 최적의 간호를 제공하기 위해 협력하는 관계를 의미한다(Choi & Bang, 2013). Choi와 Bang (2013)은 간호사-부모 파트너십을 상호성, 협력, 전문적 지식과 기술, 의사소통, 민감성, 정보공유 및 조심성으로 구성된 개념으로 설명하며, 이러한 파트너십이 아동의 돌봄과 회복에 중요한 요소라고 보고하였다.
아동 간호에서는 아동을 독립적 개체로 보기보다 가족 맥락 속에서 가족 구성원과 상호작용하는 존재로 이해해야 한다(Zgambo et al., 2025). 특히 중증 질환을 가진 아동은 병원에 반복적으로 입원하며, 부모와 가족은 아동에게 지속적으로 돌봄을 제공하는 주 보호자로서 중요한 역할을 한다. 또한 어린 아동은 자신의 상태나 요구를 표현하기 어렵기 때문에 부모가 아동의 대변자 역할을 하고, 아동의 이익을 보호하게 된다(Kim & Chang, 2025). 이러한 이유로 아동 분야에서는 아동에게 무엇이 최선인지에 대해 판단하는 데 있어서 가족의 역할을 중요한 요소로 인식하고 있다(Quaye et al., 2025). 또한 간호사가 아동의 치료 과정에서 부모가 의사결정에 참여할 수 있도록 개별적으로 접근하고, 부모를 존중하는 태도를 보일수록 부모가 느끼는 무력감과 불안을 완화하여 부모의 스트레스가 감소하는 것으로 보고되었다(Loutfy et al., 2024). 따라서 아동의 치료와 간호 과정에서 간호사와 부모가 협력적 관계를 형성하는 간호사-부모 파트너십은 중요한 개념으로 강조되고 있다.
아동 간호에서는 간호사-부모 파트너십의 의미 탐색, 측정 도구 개발, 서술적 연구 및 실험 연구 등 다양한 연구가 이루어져 왔다. 하지만 간호사-부모 파트너십 관련 연구의 국내·외 동향을 체계적으로 파악한 연구는 충분하지 않은 실정이다. Park 등(2024)은 국내에서 수행된 간호사-부모 파트너십 연구의 동향을 확인하기 위해 문헌 분석을 수행하였으나, 해당 연구는 국내 연구에 한정되어 있어 간호사-부모 파트너십에 대한 국제적인 연구 동향을 파악하는 데에는 한계가 있다. 주제 범위 문헌고찰은 연구 질문과 관련된 특징, 범위 및 핵심 개념을 요약하고 매핑하여 광범위한 연구 결과를 개괄적으로 파악하기 위한 연구방법이다(Arksey & O'Malley, 2005; Levac et al., 2010). 이에 이 연구에서는 국내·외에서 수행된 간호사-부모 파트너십 관련 연구를 체계적으로 수집하고 분석하여 현재까지 이루어진 아동 간호에서 간호사-부모 파트너십과 관련된 연구를 개괄적으로 파악하고 향후 연구 방향을 제시하기 위해 주제 범위 문헌고찰을 수행하고자 한다. 이 연구의 구체적인 목표는 국내·외 문헌에서 나타난 아동 간호에서 간호사-부모 파트너십과 관련된 요인과 간호사-부모 파트너십 형성과 관련된 요소를 파악하기 위함이다. 이를 통해 궁극적으로 향후 연구의 방향을 모색하는 데에 기여하고자 한다.

대상 및 방법

1. 연구 설계

이 연구는 국내 및 국외에서 이루어진 간호사-부모 파트너십에 관한 연구의 동향을 알아보기 위한 주제 범위 문헌고찰 연구이다. 이 연구는 Arksey와 O'Malley (2005)가 처음 제시하고, Levac 등(2010)이 보완한 주제 범위 문헌고찰 방법론을 따라 연구 질문 정하기, 관련 연구 확인하기, 연구 선택하기, 자료 추출하기, 요약 및 결과 보고하기의 5단계로 수행하였다. 이 연구는 주제 범위 문헌고찰의 특성 상 포함 문헌의 질 평가는 수행하지 않았다. 또한 the Joanna Briggs Institute (JBI)에서 제시한 지침을 참고하여 연구를 진행하였다(Peters et al., 2015). 이 연구는 PRISMA-ScR (the Preferred Reporting Items for Systematic Reviews and Meta-Analyses extension for Scoping Reviews) 체크리스트(Tricco et al., 2018)에 따라 보고하였다.

2. 연구 절차

1) 연구 질문 정하기

주제 범위 문헌고찰에서 연구 질문은 주제 범위 문헌고찰의 목적을 고려하여 명확하게 기술되어야 한다(Arksey & O’ Malley, 2005; Levac et al., 2010). JBI 지침(Peters et al., 2015)에 따라, 이 연구의 연구 질문은 인구집단-개념-맥락(Population-Concept-Context, PCC)을 적용하여 인구집단-간호사와 부모, 개념-파트너십, 맥락-아동간호로 구성했다. 연구 질문은 “아동간호에서 간호사와 부모 간의 파트너십과 관련된 요인과 간호사-부모 파트너십 형성에 관련된 요소는 무엇인가?”이다.

2) 관련 연구 확인하기

간호사-부모 파트너십 개념이 정립되고 관련 연구가 본격적으로 이루어진 2000년대 이후부터 연구 동향을 분석하기 위해, 문헌의 출판 기간은 2000년 1월부터 검색 시점까지로 설정하였다. 관련 논문에서 주로 사용되는 용어와 연구 개괄을 파악하기 위해 본격적인 자료 검색 전 시험적으로 검색하여 분석에 포함할 검색어를 설정하였다. 논문 검색은 2025년 2월 13일부터 2월 21일까지 시행하였으며, 국내 데이터베이스인 Research Information Sharing Service (RISS), Korean Medical Database (KMbase), KoreaMed, National Discovery for Science Library (NDSL), DBpia를 이용하였고, 국외 데이터베이스인 Pubmed, Cumulative Indexing Nursing & Allied Health Literature (CINAHL), MEDLINE, Web of Science 를 이용하였다. 연구 질문의 주요어를 중심으로 설정하였고, 이 연구에서 대상자와 관련된 검색어는 ‘Nurse’와 ‘Pediatric’, ‘Baby’, ‘Children’ 등의 간호사, 부모, 아동과 관련된 용어를 사용하였고, 개념은 ‘Partnership’을 사용하였다. 그리고 맥락 및 배경과 관련된 용어는 ‘Pediatric ward’, ‘neonatal intensive care unit (NICU)’ 등을 사용하였다. 국내·외 데이터베이스에 적용한 검색식은 연구자가 소속된 대학의 도서관 사서의 자문을 받아 각 데이터베이스의 주요 색인어, 자연어 및 Boolean 연산자를 사용하여 구성하였다. 예를 들어, RISS에서는 ‘전체’ 검색 필드를 사용하여 ‘(nurse) AND (pediatric | preterm | baby | infant | toddler | child | children) OR (pediatric | pediatric ward | PICU | NICU | pediatric outpatient | Neonatal intensive care unit | pediatric intensive care unit | pediatric emergency department | pediatric emergency ward) AND (Nurse Parent Partnership | Pediatric Nurse Parent Partnership | Nurses on Partnership with Parents | Nurse/Parent Relationships | Parent-nurse interactions)’를 적용하였으며, 발행연도는 2000년 이후로 제한하였다. 포함 기준은 동료평가를 거친 전문학술지의 원저이며 한국어 또는 영어로 출판된 문헌이다. 제외 기준은 원문을 확보할 수 없는 논문이다.

3) 연구 선택하기

국내·외 데이터베이스를 통해 일차 검색된 논문은 912건이었다. EndNote를 이용하여 중복되는 논문 289건을 제외하였고, 연구자 2인이 독립적으로 623건 논문의 제목과 초록을 확인하였다. 그리고 합의하는 과정을 거쳐 연구 질문과 관련 없는 논문 525건을 배제하였다. 이후 원문이 확인되지 않은 논문 71건을 제거하였고, 원문을 확보한 27건 중 간호사-부모 파트너십과 관련이 없는 논문이거나 도구 개발 논문 5건은 간호사-부모 파트너십과의 관계를 분석하고자 하는 연구 질문과 부합하지 않아 배제하였다. 검색 시 누락된 논문이 있을 수 있기 때문에 22건 논문의 참고문헌 목록을 확인하여 간호사-부모 파트너십과 관련된 논문 14건을 추가하였다. 연구자 2인이 추가된 논문의 원문을 확인한 결과 1건의 논문은 연구질문과 관련 없는 연구로 평가하여 배제하였다. 이에 최종적으로 35편의 논문을 분석하였고, 자료를 수집하고 추출한 과정은 Fig. 1과 같다.

4) 자료 추출, 요약 및 결과 보고

이 연구에서는 자료 분석에 앞서, 연구 주제에 맞는 자료를 추출하기 위해 JBI 지침(Peters et al., 2015)에서 제안한 정보 유형을 참고하여 예비 분석을 시행한 후에 최종적인 자료 추출 항목을 보완하였다. 이 연구에서 적용한 자료 추출 양식은 제1저자, 발행 연도, 발행 국가, 연구 설계, 연구 목적, 자료 수집 방법, 대상자 유형 및 표본 수, 연구가 수행된 장소, 주요 변수, 중재 이름, 중재 기간, 측정 도구, 연구 결과가 포함되었다. 연구자 2인이 독립적으로 최종 선정된 35편의 문헌의 자료를 정리하였고, 합의 과정을 거쳐 최종 자료를 확정하였다. 최종 선정된 35편 문헌의 주요 내용은 Table 1에 요약하였다. 이를 토대로 문헌의 일반적 특성(Table 2)과 간호사-부모 파트너십 관련 요인 및 형성 요소(Table 3)를 중심으로 분석한 결과가 도출되었다.

결 과

1. 선정된 문헌의 일반적 특성

최종 선정된 문헌의 일반적 특성은 발행 연도, 발행 국가, 연구 설계, 병원 유형, 연구가 수행된 병동 유형, 대상자 유형으로 정리하였다(Table 2). 발행 연도를 살펴보면 2002년에서 2015년까지는 25.7%가 발행되었고, 2016년에서 2020년에 45.7%의 논문이 발행되어 이 시기에 연구가 활발히 이루어진 것을 확인할 수 있었다. 2021년에서 2024년에 발행된 논문이 28.6%로 두번째로 많았다. 연구는 대한민국, 미국, 호주, 영국, 캐나다, 중국, 뉴질랜드, 사우디아라비아에서 이루어졌으며, 대한민국에서 68.5%로 가장 많았다. 연구 설계는 서술적 연구 65.7%, 질적 연구 11.4%, 실험 연구 8.6%, 혼합 연구 5.7%, 이론 연구 5.7%, 문헌고찰 연구 2.9%가 이루어졌다. 상급종합병원 및 종합병원에서 수행된 연구가 34.3%, 상급종합병원 및 종합병원, 어린이병원에서 수행된 연구가 22.8%, 어린이병원에서만 수행된 연구가 14.3%였다. 연구가 이루어진 병동은 다양했는데, 소아 병동에서 이루어진 연구가 31.4%, 여러 병동에서 이루어진 연구가 20%, 신생아중환자실 17.1%, 소아심장중환자실 8.6%, 소아중환자실 2.9%였다. 대상자 유형은 간호사, 어머니 또는 부모, 의사, 아동이었는데, 간호사만 대상으로 한 연구가 57.1%로 가장 많았고, 어머니 또는 부모만 대상으로 한 연구가 17%이었다.

2. 간호사-부모 파트너십에 영향을 미치는 요인

간호사-부모 파트너십에 영향을 미치는 요인을 간호사 관련 요인, 부모 관련 요인으로 분류하였다(Table 3). 간호사-부모 파트너십에 영향을 미치는 간호사와 관련된 심리사회적 요인은 4편의 논문에서 자기효능감(R8, R16, R22, R26), 3편의 논문에서 임상적의사결정(R8, R15, R35), 3편의 논문에서 임파워먼트(R8, R14, R29), 5편의 논문에서 의사소통 역량, 기술 및 유형(R26, R27, R28, R29, R33), 감정노동(R16), 가족의 중요성에 대한 태도(R22), 대인관계 역량(R29), 3편의 논문에서 감성지능(R28, R29, R33)으로 확인되었다. 간호사의 사회인구학적 요소와 관련한 요인으로는 3편의 논문에서 연령(R24, R27, R29), 4편의 논문에서 결혼 상태(R8, R16, R24, R26), 2편의 논문에서 종교(R6, R11), 3편의 논문에서 근무부서 만족도(R8, R13, R26), 2편의 논문에서 담당 환자 수(R14, R26), 5편의 논문에서 임상 경력(R11, R14, R16, R22, R27), 소아 관련 추가 자격 이수 여부(R33), 석사 이상의 학력(R8), 2편의 논문에서 소아관련 병동 유형(R6, R14)이 간호사-부모 파트너십에 영향을 미치는 것으로 확인되었다. 아동 부모와 관련된 요인은 입원 아동의 진단(R7)과 입원 횟수(R13)였다.

3. 간호사-부모 파트너십과의 상관관계

간호사-부모 파트너십과 상관관계가 있는 요인은 간호사 관련 요인과 부모 관련 요인이 있다(Table 3). 간호사의 자기효능감이 3편의 논문(R6, R9, R23)에서 간호사-부모 파트너십과 상관관계를 보였다. 그리고 감정노동(R9), 감성지능(R23), 간호전문직관(R6), 재직의도(R30), 피로(R34)가 상관관계가 있는 것으로 확인되었다. 부모 측면에서는 아동 부모의 대처(R7)와 부모에 대한 사회적 지지(R21)가 간호사-부모 파트너십과 상관관계가 있는 것으로 보고되었다.

4. 간호사-부모 파트너십이 미치는 영향

여러 연구에서 간호사-부모 파트너십이 다양한 요인에 영향을 미치는 것을 확인하였다. 이 연구에서는 간호사-부모 파트너십의 영향을 받는 요인을 간호사 관련 요인과 부모 관련 요인으로 분류하여 제시하였다(Table 3). 간호사와 관련된 결과로는 간호사-부모 파트너십이 간호의 질에 영향을 미치는 것을 3편의 논문(R13, R22, R35)에서 확인하였고, 3편의 논문에서 직무만족도(R15, R18, R28), 직무 스트레스(R9), 간호 역량(R23)에 영향을 미치는 것으로 보고되었다. 부모와 관련된 결과로는 부모의 불안이 간호사-부모 파트너십의 영향을 받는 것을 2편의 논문(R13, R19)에서 확인하였다. 그리고 간호사-부모 파트너십이 부모 만족도(R19), 양육 자기효능감(R19), 퇴원 준비도(R21)에 영향을 미쳤다.

5. 간호사-부모 파트너십 형성과 관련된 요소

선정된 문헌 중 질적연구, 혼합 연구, 개념 분석 연구, 실험 연구 9편에서 간호사-부모 파트너십의 구성 요소를 확인하였다(Table 3). R1와 R3 논문에서 부모가 자신의 돌봄 제공 능력을 전문가에게 인정받을 때 파트너십이 향상되는 것으로 보고되었다. 그리고 간호사가 아동에 관해서는 부모가 전문가라고 생각하고, 부모에게 조언을 구하는 행동을 하는 가족에 대한 존중(R3, R10)이 파트너십을 형성하는데 중요한 요소로 확인되었다. 위기 상황에서 간호사가 부모를 지지하면 파트너십을 더 강화할 수 있었다(R1, R3, R10, R31). 간호사가 부모에게 경청하고, 정보를 명확하고 공개적으로 전달하는 간호사와 부모 사이의 의사소통이 중요한 구성요소로 보고되었다(R3, R4, R10, R31). 간호사와 부모 사이의 빈번하고 지속적인 상호작용을 통한 관계 형성 또한 중요했다(R1, R3, R12, R31). R1과 R12 논문에서 정보 공유의 중요성을 강조했다. 간호사가 부모에게 정서적 지지를 제공하면 파트너십을 더 강화할 수 있었다고 보고된다(R1, R10). 간호사가 아동과 부모에게 개별화된 간호를 제공하고, 섬세하게 접근하면 파트너십이 향상되었다(R1, R12). 부모가 돌봄에 참여하는 것이 파트너십을 형성하는데 중요한 것으로 보였다(R17, R19, R25).

고 찰

이 연구는 국내외의 간호사-부모 파트너십과 관련된 연구 경향을 알아보기 위해 간호사-부모 파트너십과 관련된 35편의 문헌을 주제 범위 문헌 고찰하였다. 분석한 결과, 간호사-부모 파트너십 관련 연구는 2016년 이후 증가했다. 연구 유형은 질적 연구, 이론 연구, 실험 연구 등 다양한 연구가 이루어졌지만, 그 중에서 서술적 연구가 가장 많이 이루어졌다. 또한, 간호사-부모 파트너십과 관련된 다양한 요인이 보고되었으나, 일부 요인은 간호사-부모 파트너십과 상관관계는 있지만 인과관계가 확인되지 않았다. 반면에, 간호사-부모 파트너십이 간호 역량에 영향을 주는 것으로 확인되었으나 인과관계를 명확히 할 수 없는 것으로 보인다. 이 연구에서 간호사-부모 파트너십 형성과 관련된 다양한 요소를 확인하였으며, 간호사-부모 파트너십과 관련이 있는 부모 요인은 간호사 요인보다 상대적으로 적었다. 이와 같이 도출된 연구 결과를 토대로 간호사-부모 파트너십 연구 경향과 한계를 논의하고자 한다.
간호사-부모 파트너십 관련 연구는 2015년까지는 활발하지 않았으나, 2016년 이후로 급격히 증가하였다. 간호학 연구에서는 질적 연구를 통해 현상을 탐색하고, 개념을 이론적으로 정립하며, 현상의 양적 분석을 위해 도구를 개발하는데(Polit & Beck, 2021), 2010년까지는 질적연구와 파트너십 의미 탐색 연구가 주로 이루어진 것으로 보인다. 2013년에 한국에서 간호사와 부모 사이의 파트너십을 측정할 수 있는 도구인 간호사-부모 파트너십 도구(Pediatric Nurse Parent Partnership Scale, PNPPS)가 개발되었다(Choi & Bang, 2013). 도구가 개발된 이후 임상에서 파트너십을 실질적으로 측정하고 분석이 가능해져 2016년 이후로 간호사-부모 파트너십 도구를 이용한 서술적 연구가 증가한 것으로 보인다.
이 연구에서 분석한 35편의 간호사-부모 파트너십 연구 중 서술적 연구가 큰 비중을 차지하였으며, 이에 비해 실험연구는 제한적인 것으로 나타났다. 선행연구(Park et al., 2024)에서도 이와 유사하게 국내에서 수행한 파트너십 연구 30편을 분석한 결과로 80%가 서술적 연구였고, 6.7%만이 실험 연구였다. 이는 국내 및 국외에서 파트너십 연구가 주로 파트너십의 수준과 관련 요인을 파악하는 초기 단계의 연구에 집중되어 왔기 때문으로 생각된다. 현상을 이해하기 위해 수행된 양적 연구 결과는 충분히 축적되어 있으나, 이를 기반으로 한 중재 프로그램의 개발과 그 효과를 검증하는 연구로의 확장은 아직 활발하게 이루어지지 않은 것으로 보인다. 여러 연구에서는 환아 부모와의 파트너십을 향상하기 위한 교육 프로그램 개발의 필요성을 제기한 바 있다(Cho & Han, 2024; Cho & Park, 2022; Lee & Han, 2024). 따라서 향후에는 간호사와 부모의 파트너십을 향상할 수 있는 교육 프로그램을 포함한 중재 모델을 개발하고, 중재 모델의 효과를 검증하는 실험 연구가 보다 활발히 이루어질 필요가 있다.
이 연구의 결과, 간호사-부모 파트너십과 유의한 상관관계가 있는 요인 중 자기효능감(Choi & Kim, 2014; Kim & Lee, 2021; Yoo & Cho, 2020; Youn & Noh, 2018), 감정노동(Youn & Noh, 2018), 감성지능(Kim et al., 2023; Lee & Choi, 2023; Paulsamy et al., 2024)은 파트너십의 영향 요인으로 제시되었으나, 간호전문직관(Cho & Bang, 2013), 재직의도(Ji et al., 2024), 간호사의 피로(Jeon et al., 2024)는 상관관계 수준에서만 확인되었다. 이는 파트너십이 간호사의 개인적 역량 및 직무와 관련된 심리·정서적 특성에 영향을 받을 가능성을 보여준다. 반면, 간호전문직관, 재직의도, 피로는 상관관계 수준에서만 보고되어 간호사-부모 파트너십 간의 인과적 관계를 확인하기에는 한계가 있었다. 임상 현장에서 파트너십을 향상시키기 위해 어떤 요인을 개발해야 하고 중재해야 하는지에 대한 근거가 아직 부족하다고 볼 수 있다. 여러 선행연구에서 간호전문직관이 간호 역량 정도를 향상시키는 데에 유의한 영향 요인으로 보고되고 있다(Cho & Ha, 2024; Grande et al., 2022; Im & Ha, 2025). 이를 바탕으로 아동 간호사의 간호전문직관과 간호 역량의 관계에서 간호사-부모 파트너십의 매개 효과를 확인해볼 필요가 있다. 또한 선행연구에서 간호의 질과 간호 수행을 개선하기 위해서는 간호사의 피로 관리가 중요하다고 보고되고 있다(Bell et al., 2023; Cho et al., 2022). 하지만 이 연구 결과 아동 간호사의 피로를 다룬 연구는 한 편에 그친 것으로 나타났다(Jeon et al., 2024). 따라서 파트너십 향상을 위한 중재 프로그램 개발을 위해서는 간호전문직관, 재직의도, 피로와 인과관계를 규명하는 연구가 반복적으로 필요할 것으로 보인다.
이 연구에서 도출된 결과를 종합한 결과, 간호의 질(Bae & Lee, 2017; Lee & Choi, 2024; Yoo & Cho, 2020)과 간호 역량(Mun & Yoo, 2020)은 간호사-부모 파트너십과 밀접한 관련이 있었으며, 간호사-부모 파트너십은 간호의 질과 간호 역량을 향상시키는 데에 긍정적인 영향을 미치는 것으로 확인되었다. 선행연구에서도 간호사-부모 파트너십 수준이 높을수록 간호 역량의 수준이 높았으며 높은 간호 역량은 부모의 참여를 증진하는 것으로 보고되어(Dehkordi et al., 2023), 이 연구 결과를 뒷받침하였다. 이러한 결과는 아동 간호에서 부모와의 파트너십 형성이 중요한 요소임을 시사한다. 한편, 이 연구에서 도출된 결과, 임상적 의사결정 능력(Choi & Kim, 2014; Lee & Choi, 2024; Shin & Kim, 2018), 의사소통 능력(Cho & Park, 2022; Kim et al., 2023; Kim & Lee, 2021; Lee & Choi, 2023; Paulsamy et al., 2024), 대인관계 능력(Kim et al., 2023)과 같은 간호사의 전문직 수행과 관련된 역량이 간호사-부모 파트너십에 영향을 주는 것으로 나타났다. 이는 파트너십이 간호 역량에 영향을 미칠 뿐 아니라, 간호 수행과 관련된 간호사 개인의 역량 또한 파트너십에 영향을 미칠 가능성을 보여준다. 이 연구의 결과 간호사-부모 파트너십과 간호 역량의 관계를 파악한 연구는 한 편에 그치고 있어(Mun & Yoo, 2020), 간호사-부모 파트너십과 간호 역량의 두 변수 간에 인과관계가 명확하지 않은 실정이다. 아동 간호에 있어 간호 역량의 의미가 명확하지 않아 연구마다 간호 역량의 의미에 따라 다르게 나타날 수 있다. 따라서 간호 역량의 의미를 명확히 하여 간호사-부모 파트너십과의 인과관계를 검증하는 연구가 반복적으로 필요하다.
이 연구에서는 간호사-부모 파트너십을 형성하기 위하여 필요한 요소로 부모를 돌봄 제공자로 인정(Bruns & McCollum, 2002; Lee, 2007), 부모에 대한 존중(Lee, 2007; Mendes, 2016), 부모 지지(Barratt et al., 2024; Bruns & McCollum, 2002; Lee, 2007; Mendes, 2016), 간호사 부모 간의 의사소통과 관계 형성(Barratt et al., 2024; Hopwood et al., 2018; Lee, 2007; Plumridge et al., 2009), 정보 공유(Bruns & McCollum, 2002; Hopwood et al., 2018), 정서적 지지(Bruns & McCollum, 2002; Mendes, 2016), 개별화된 간호(Bruns & McCollum, 2002; Hopwood et al., 2018), 아동 돌봄에 부모 참여(Heo & Oh, 2019; Klug et al., 2020; Uhm & Kim, 2019)를 확인하였다. 이는 Choi와 Bang (2013)이 개발한 PNPPS의 하위 요인과 유사하다. PNPPS의 하위 요인으로는 간호사와 부모가 동등한 파트너로서 역할을 수행하는 상호성, 아이 돌봄에 부모와 간호사가 공동으로 참여하는 협력, 간호사가 부모를 돌봄 전문가로 인식하는 존중, 아동 간호사와 부모, 의료진 간의 의사소통과 관련된 내용을 포함하는 의사소통, 아동 간호사와 부모 간의 정보를 교환하는 정보 공유가 포함된다(Choi & Bang, 2013). 이 연구에서 간호사-부모 파트너십 도구의 하위 요인 외에 부모 정서적 지지(Bruns & McCollum, 2002; Mendes, 2016), 개별화된 간호(Bruns & McCollum, 2002; Hopwood et al., 2018)가 추가적으로 확인되었다. Cho와 Oh (2023)의 연구에서는 간호사와 부모 간 파트너십을 발달시키기 위해서는 입원 초기 단계에서부터 부모에 대한 정서적 지지가 제공하여 신뢰 관계를 형성하여야 한다고 보고한다. 또한, Bruns와 McCollum (2002)은 부모에 대한 개별적 접근이 부모에게 자신감을 주고, 부모에게 미숙아를 돌보는 실질적 방법과 의료진과 소통하는 법을 제공한다고 설명한다. 이 연구에서 도출된 결과를 종합하면 간호사-부모 파트너십이 다양한 치료적 상황 속에서 형성되며, 의료진과 부모가 공동의 돌봄 목표를 중심으로 상호작용을 하는 협력적 관계를 포함하는 복합 개념이라고 볼 수 있다. 현재까지 복합 개념인 파트너십을 설명하고, 관련 요인을 규명하기 위해 지속적으로 연구가 이루어져 왔다. 이러한 연구를 기반으로 향후 파트너십 향상시키기 위한 프로그램은 선행연구에서 규명된 다양한 관련 요인을 반영하여 개발할 필요가 있다.
이 연구 결과, 부모 관련 요인은 간호사 요인에 비해 제한적인 것으로 나타났다. 또한, 파트너십에 영향을 미치는 부모 관련 요인도 부모의 특성이기보다는 입원 아동의 진단(Cho & Lee, 2013), 입원 횟수(Bae & Lee, 2017)와 같은 입원 관련 특성이었다. 이 연구에서 분석한 결과 35편의 연구 중 부모를 연구 대상으로 포함한 연구는 37.1%였고, 그중 환아 어머니 또는 부모만을 대상으로 한 연구는 17%에 그쳤다. 이러한 결과를 보았을 때 부모를 대상으로 한 연구가 충분하지 않았기 때문에 파트너십과 관련된 부모 관련 요인이 제한적이었던 것으로 보인다. 간호사-부모 파트너십의 기반이 되는 FCC는 의료진과 가족 간의 협력을 통해 공동으로 의사결정을 이루는 과정을 전제로 하며, 부모는 자녀 간호에서 자신의 참여 방식과 역할을 의료진과 협의하여 결정해야한다(Razeq et al., 2024). 또한, FCC 실현은 의료진과 부모의 공고한 파트너십을 바탕으로 부모가 실제 돌봄에 참여하고, 자신의 참여를 어떻게 인식하는지에 따라 달라진다(Almalki et al., 2025). 따라서 파트너십을 형성하는 데는 간호사뿐 아니라 부모의 역할도 중요하기 때문에 환아 부모를 대상으로 부모 관련 특성을 확인하고, 파트너십과 관련된 요인을 파악하는 후속 연구의 반복이 필요하다.
이 연구의 제한점을 바탕으로 다음과 같이 제언한다. 첫째, 부모가 상주하는 소아 병동과 부모가 상주하지 않는 중환자실은 간호사와 부모의 파트너십의 정도나 이에 영향을 미치는 요인이 다를 수 있으므로 병동별 차이를 비교하고 분석하는 연구가 필요하다. 둘째, 이 연구에서는 파트너십 개발 프로그램을 구체적으로 분석하지 않았기에 현재까지 개발된 프로그램의 구성 요소와 효과를 체계적으로 분석하는 연구가 필요하다. 셋째, 이 연구는 특정 언어로 제한하여 문헌을 검색하였기 때문에 일부 관련 연구가 포함되지 않았을 가능성이 있다. 따라서 향후에는 다양한 언어를 포함한 문헌 검색을 통한 연구가 이루어질 것을 제언한다.

결 론

이 연구는 간호사-부모 파트너십 관련 문헌 35편을 주제 범위 문헌고찰을 통해 분석하여 연구 동향을 파악하고 향후 연구 방향을 제시하였다. 분석 결과, 간호사-부모 파트너십 연구는 주로 서술적 연구 중심으로 이루어져 왔으며, 이를 기반으로 한 중재 프로그램 개발 및 효과 검증 연구는 제한적인 것으로 나타났다. 이에 따라 향후 연구에서는 간호사와 부모의 파트너십 향상을 위한 중재 프로그램을 개발하고 그 효과를 검증하는 실험연구가 확대될 필요가 있다. 또한 파트너십 형성에서 부모의 역할이 중요함을 고려하여 부모를 포함한 연구를 축적하고, 간호전문직관, 재직의도, 피로 등 다양한 관련 요인을 포함한 확장 연구를 통해 파트너십의 선행 요인과 결과 요인을 체계적으로 규명할 필요가 있다.
